Zusammenfassung:
1 Zusammenfassung 1.1 Hintergrund und Ziele Einer Erkrankung der Pulpa und des periapikalen Gewebes liegt immer eine mikrobielle Infektion des Endodonts zu Grunde. Mikroorganismen verbleiben in Dentintubuli, akzessorischen Foramina, Isthmen und lateralen Kanälen. Da die Irregularitäten und laterale Kanäle mit aktuellen Behandlungsprotokollen nur unzureichend gereinigt werden können, besteht das Ziel einer endodontischen Therapie darin, im Anschluss an die chemo-mechanische Reinigung, das gesamte, komplexe Wurzelkanalsystem dreidimensional zu versiegeln um somit eine Reinfektion zu verhindern. Das Ziel dieser In-vitro-Studie war es, vier verschiedene Sealer-Placementtechniken auf Penetrationstiefe, Homogenität und Vollständigkeit des Sealers in simulierte Seitenkanäle und Mulden zu untersuchen. 1.2 Methode An 60 humanen, extrahierten Zähnen mit geraden Wurzeln und nur einem Wurzelkanal wurden, auf Höhe der Schmelz-Zementgrenze, die Kronen abgetrennt. Die Aufbereitung erfolgte maschinell bis Größe ISO 70 mit dem FlexMaster-System (VDW, München, Deutschland) und anschließend bis ISO 80 mit einer Mity Roto-File (Loser & Co, Leverkusen, Deutschland). Anschließend wurden die Wurzeln der Länge nach so getrennt, dass zwei identische Hälften entstanden. Eine Hälfte wurde mit einer Rille mit einer Länge von 6 mm und 2 mm Tiefe 2 mm vom Apex entfernt versehen. In die zweite Hälfte wurden 4 Mulden im Abstand von 2 mm, 4 mm, 6 mm und 8 mm vom Apex entfernt, mit 2 mm Tiefe, präpariert. Die so vorbereiteten Wurzeln wurden zusammengesetzt, in Biresin-Polyurethanblöcke (Sika N.V, Utrecht, Niederlande) eingebettet. 1 Zusammenfassung 2 Die Proben wurden in 4 Gruppen mit jeweils 15 Zähnen entsprechend der Sealerapplikationstechnik aufgeteilt: Gruppe 1: mit einem Guttaperchapoint, Gruppe 2: mit einer Papierspitze, 3: schallaktiviert mit einer ROEKO CanalBrush (Coltène/Whaledent AG, Altstätten, Schweiz), Gruppe 4: mit Ultraschall (EMS 600) und einem Spreaderansatz. Die Wurzelkanalfüllung erfolgte mit den obengenannten Applikationstechniken und einen auf „Tug-Back“ und dem Kanalquerschnitt kongruenten Guttaperchapoint mit Hilfe der Single-cone-Technik. Als Sealer wurde AH Plus (Dentsply DeTrey GmbH, Konstanz, Deutschland) verwendet. Nach 48-stündiger Aushärtung wurden die Proben in oro-vestibulärer Richtung anhand von digitalen Röntgenbildern (Digora, Soredex, Helsinki, Finnland), auf Penetrationstiefe, Homogenität und vollständiges Ausfließen des Sealers ausgewertet. 1.3 Ergebnisse Die schall- bzw. ultraschallaktivierten Gruppen zeigten hinsichtlich Homogenität, Penetrationstiefe und Vollständigkeit signifikant bessere Werte verglichen mit den Gruppen in denen der Sealer mit einem Guttaperchastift oder einer Papierspitze eingebracht wurde. Die Ultraschallgruppe zeigte die besten Ergebnisse. Zwischen der Gruppe 1 und Gruppe 2 gab es statistisch keinen Unterschied. 1.4 Schlussfolgerung Die Auswertung der Ergebnisse dieser In-vitro-Arbeit zeigte, dass eine Aktivierung des Sealers, sowohl mit Schall als auch mit Ultraschall, der herkömmlichen, manuellen Sealerplacement-Technik signifikant überlegen ist, was die Penetrationstiefe, Homogenität und Vollständigkeit in simulierte laterale Kanäle und Irregularitäten betrifft.
