Zusammenfassung:
1.1 Hintergrund und Ziele Die komplexe Wurzelanatomie und die vielseitige Morphologie eines Zahnes stellen sogar den erfahrenen Endodontisten immer wieder vor das Problem, eine dichte und adäquate Obturation dieses Systems mit seinen zahlreichen Foramina, akzessorischen und lateralen Seitenkanälen zu bewältigen. Um die Voraussetzungen für eine Heilung des apikalen Gewebes zu schaffen, ist es von großer Bedeutung, nach einer vollständigen Reinigung und Desinfek-tion einen hermetischen Verschluss zu gewährleisten, damit sich keine Mög-lichkeiten mehr für den Austausch von Gewebszerfallsprodukten und Bakte-rien mit dem umliegenden Gewebe ergeben. Aus diesem Grund sollte das Abfüllen von Seitenkanälen bei einer Wurzel-behandlung den gleichen Stellenwert einnehmen wie die Obturation des Hauptkanals, um so eine Reinfektion des Kanals durch bakterielle Residuen zu verhindern. Das Ziel dieser In-vitro-Studie war es, die Effizienz von vier verschiedenen Sealer-Applikationstechniken hinsichtlich Penetrationstiefe und Homogenität des Sealers AH Plus in simulierten Seitenkanälchen zu messen. 1.2 Methode Bei 60 humanen, extrahierten, einwurzeligen Zähnen wurden die Kronen ab-getrennt, die Wurzeln danach longitudinal gespalten und in Polyurethanblö-cke (Biresin, Sika N.V, Utrecht, Niederlande) eingebettet. Die Präparate wur-den in 4 Gruppen mit jeweils 15 Zähnen entsprechend der verschiedenen Plazierungsverfahren für Sealer aufgeteilt: Gruppe 1: rotierend mit einer EZ-Fill bi-directional spiral (EDS, South Hackensack, New Jersey, USA), Grup-pe 2: rotierend mit einem K-Räumer (Dentsply Maillefer, Ballaigues, Schweiz) der Größe 70, Gruppe 3: rotierend mit einem Lentulo der Größe 40 (VDW, München, Deutschland), Gruppe 4: rotierend mit einer ROEKO CanalBrush (Coltène/Whaledent, Altstätten, Schweiz). Die maschinelle, vollrotierende Aufbereitung erfolgte zuerst mit FlexMaster-Instrumenten (VDW, München, Deutschland) auf Größe 70 Taper .02 und anschließend mit einer Mity Roto-Feile ISO 80 (Loser & Co, Leverkusen, Deutschland). Im nächsten Schritt erfolgte das Anlegen einer 6 mm langen und 2 mm tiefen Rille auf einer der Wurzelhälften, welche 2 mm vom Apex entfernt war. Auf der zweiten Hälfte wurden vier Mulden im Abstand von 2 mm, 4 mm, 6 mm und 8 mm, ebenfalls mit einer Tiefe von 2 mm vor Apex präpariert. Danach erfolgte die Wurzelkanalfüllung mit AH Plus (Dentsply DeTrey, Konstanz, Deutschland) und den oben genannten Applikationstech-niken. Nach 48-stündiger Aushärtung wurden digitale Röntgenbilder (Digora, Soredex, Helsinki, Finnland) in oro-vestibulärer Projektion angefertigt, um die Penetrationstiefe und Homogenität der Füllung zu beurteilen. 1.3 Ergebnisse Es konnten in dieser Studie keine signifikanten Unterschiede in der Obturati-on von artifiziell angelegten Seitenkanälen mit den oben genannten Applika-tionstechniken ermittelt werden. Die Studie zeigte allerdings, dass die koro-nal lokalisierten Mulden und Rillen eine tiefere Penetration durch Sealer auf-wiesen als die apikalen Anteile. Demnach konnten koronal maximal 33,3 % der Mulden vollständig abgefüllt werden und apikal maximal 20 %. 1.4. Schlussfolgerung Bei Betrachtung aller Gruppen dieser Studie kann festgehalten werden, dass bei allen rotierenden Sealer-Applikationsmethoden keine signifikanten Unter-schiede gemessen werden konnten.
