Transkriptom- und Proteom-Analyse der Iris bei Kindern mit juveniler idiopathischer Arthritis-assoziierter Uveitis

In der vorliegenden experimentellen Arbeit wurde initial mittels RNA-Seq das Genexpressionsprofil in der Iris bei Patienten mit JIAU analysiert mit dem Ziel, diejenigen Gene zu identifizieren, die im Vergleich zur Kontroll-Gruppe signifikant DE waren. Dabei wurde zunächst mittels der Cluster-Analyse festgstellt, dass die analysierten Proben aufgrund ihres Genexpressionsprofils zwei Gruppen bildeten, bestehend aus den Proben der JIAU-Gruppe und den Proben der Kontroll-Gruppe. Mit der differentiellen Genexpressionsanalyse wurden 136 DE-Gene zwischen der JIAU- und Kontroll-Gruppe identifiziert. Innerhalb dieser 136 DE Gene konnte mittels der Anreicherungsanalyse eine signifikante Anreicherung von Prozessen festgestellt werden, die mit Angiogenese und Vaskulogense assoziiert sind. Im speziellen handelte es sich um Gene wie Kdr, Tie1, Tek und Angpt-1. Mittels qRT-PCR, die der Validierung der RNA-Seq Daten diente, konnte die signifikant erhöhte Expression ausgewählter Angiogenese-assoziierter Gene teilweise bestätigt werden. Im Kammerwasser und Serum von JIAU-Patienten wurde eine erhöhte ANGPT-2 Konzentration festgestellt. Die anschließend durchgeführte Protein-Analyse mittels LC / MS ermittelte insgesamt 54 DE-Proteine zwischen der JIAU- und Kontroll-Gruppe, wobei hier eine signifikant differentielle Expression von Proteinen der EZM in der JIAU-Gruppe festgestellt werden konnte, deren bedeutende Rolle bei Angiogenese ebenso diskutiert wurde. Mittels Western-Blot konnte eine erhöhte Expression der angiogenen Faktoren ANGPTT-1, Decorin und Lumican in der JIAU-Gruppe im Vergleich zur Kontroll-Gruppe bestätigt werden. Während in der Iris von JIAU-Patienten spaltlampenmikroskopisch in der Regel keine Angiogenese beobachtet werden kann, ist die erhöht festgestellte Expression angiogener Faktoren ggf. auch auf eine Endothelzell-Aktivierung während inflammatorischer Prozesse zurückzuführen. Betrachtete man neben der Anreicherungsanalyse diejenigen Gene mit der stärksten DE in der JIAU-Gruppe, so konnte eine signifikant erhöhte Expression zahlreicher Ig-Domänen der vaiablen und konstanten Region von Ig-Ketten in der Iris von JIAU-Patienten festgestellt werden, deren Expression auf AK-sezernierende Plasmazellen zurückzuführen ist. Die signifikant erhöhte Expression der B-Zell assoziierten TF Ebf1 und Id3 gab weitere Hinweise auf eine Anwesenheit von B-Zellen in der Iris von JIAU-Patienten. Auch hier konnte mittels qRT-PCR die signifikant erhöhte Expression teils bestätigt werden. Auf Protein-Ebene konnten mit der signifikant erhöhten Expression von Ig-Domänen sowie des MZ- und B1-spezifischen Proteins MZB1 ebenso Hinweise auf eine intraokulare Anwesenheit von B-Zellen bei JIAU-Patienten gegeben werden. Die erhöhte MZB1-Expression in der JIAU-Gruppe im Vergleich zur Kontroll-Gruppe konnte sowohl mittels Western-Blot als auch auf Genexpressionsebene mittels qRT-PCR bestätigt werden. Durch die Phänotyp-Charakterisierung möglicher intraokularer B-Zellen mittels qRT-PCR konnten durch die signifikant erhöhte Expression der B-Zell-Marker CD19 und CD20 sowie der Plasmazell-Marker CD38 und CD138 in der Iris von JIAU-Patienten die Hinweise bekräftigt werden. Ebenso gab die signifikant erhöhte Expression von IL-6 und IL-10 in der Iris von JIAU-Patienten Hinweise auf ein Plasmazell begünstigendes Milieu. Durch die Zytokinquantifizierung im Kammerwasser von JIAU-Patienten konnte festgestellt werden, dass insbesondere durch die lokal erhöhte Expression von BAFF, APRIL und IL-6 ein geeignetes Milieu für die Differenzierung und das Überleben von Plasmazellen gegeben ist, wobei nach den Ergebnissen der Studie von einem charakteristischen Indiz bei unterschiedlichen Entzündungsreaktionen im vorderen Augensegment ausgegangen werden kann.

The present study initially aimed to identify significantly DE genes by using RNA-Seq analysis in the iris of patients with JIAU compared to the control group. Based on their gene expression profile, the analysed JIAU and control samples were clearly separated into two clusters. The differential gene expression analysis identified 136 significantly DE genes in the JIAU group compared to the control group. Enrichment analysis of this gene set showed a significant enrichment of angiogenesis and vasculogenesis associated processes in the JIAU group. In detail, genes highly associated to these processes were Kdr, Tie1, Tek, and Angpt-1. For RNA-Seq validation, qRT-PCR analysis was performed and a significantly higher expression of some selected genes could partially be confirmed. Subsequently performed protein analysis using LC / MS identified 54 significantly DE proteins between the JIAU and control group. Here, a significantly differential expression of extracellular matrix proteins in the JIAU group could be observed, implying an important role in the context of aniogenesis. As compared to the control group, a higher protein expression of ANPT-1, Lumican and Decorin could be detected in the JIAU group by western blotting. Since there exist no data that account for altered angiogenic processes in the iris of JIAU patients, the observed higher expression of angiogenic factors in the iris of JIAU patients is presumably caused by an activation of the endothelium during inflammatory processes of JIAU. Regarding the mostly intense DE genes in the JIAU group, a significantly higher expression of several plasma cell specific Ig domains of variable and constant Ig chains could be observed. Additionally, the significant upregulation of the B cell specific TF Ebf1 and Id3 indicate an intraocular presence of B cells in the iris of JIAU patients. By using qRT-PCR, the higher expression of Ebf1 and Id3 could be partially confirmed. LC / MS protein analysis also revealed a significant upregulation of distinct Ig domains as well as the MZ- and B1-cell specific protein MZB1, whose higher expression in the JIAU group could be confirmed by western blotting and analyzing the gene expression analysis using qRT-PCR. Additional hints of intraocularly occuring B cells and especially plasma cells in the iris of JIAU patients raise of the phenotype charaterization using qRT-PCR, where a significant upregulation of the pan B cell markers CD19 and CD20 as well as the plasma cell markers CD38 und CD138 could be detected. Furthermore, a significantly higher IL-6 and IL-10 expression could be detected in the iris of JIAU patients compared to the control group, suggesting a favouring milieu for plasma cell differentiation. The local upregulation of BAFF, APRIL, and IL-6 in the aqueous humor from JIAU patients supports a suitable environment for plasma cell differentiation and survival. Presumably, this milieu is a specific indication for inflammatory reaction sites in the anterior eye segment.
