Der Einfluss traumatischer Ereignisse, zerstörter Grundannahmen über das Selbst- und Weltverständnis und existentieller Frustration auf wichtige psychische Ressourcen betroffener Personen

In einer Querschnittstudie wurden 46 Patienten mit einer gesicherten PTBS Diagnose und komorbiden Störungen mit zwei Kontrollgruppen (Psychotherapeuten, n=56; Blutspender, n=56) verglichen. Untersucht wurde der Einfluß verschiedener Traumaklassen auf die Grundannahmen über das Selbst und Weltverständnis, die existentielle Frustration, Kohärenzsinn, soziale Unterstützung, personale, soziale und strukturelle Ressourcen sowie die posttraumatische wie allgemeine Symptomatik. Erstmals wird eine differenzierte Beschreibung der Erschütterung der Grundannahmen in Abhängigkeit von der Art des Traumas ermöglicht (sexuelle und physische Gewalt, non-man-made-disasters und Tod einer wichtigen Bezugsperson). Art und Stärke der Traumatisierung sind mit spezifischen Ressourcen- und Symptommustern (insbesondere Dissoziation, Übererregung und Psychotizismus) verbunden. Die Opfer sexueller Gewalt zeigen dabei die geringste Ressourcenausstattung und die stärkste Symptombelastung, gefolgt von den Opfern physischer Gewalt und den beiden non-intentionalen Traumagruppen. Abschließend erfolgt eine Diskussion des wissenschaftlichen und psychotherapeutischen Nutzens der Ergebnisse unter Berücksichtigung der Einschränkungen einer Querschnittuntersuchung.

A cross-sectional-study compared 46 patients with a PTSD diagnosis and comorbide disorders with two groups of contols (psychotherapists, n=56, blood donators, n=56). The aim was the examination of the impact of different traumatic events on world of assumptions, existential frustration (i.e. lack of meaning), sense of coherence, social support, personal, social, and structural ressources, and posttraumatic and other symptoms. For the first time an exact description of "shattered assumptions" becomes avalilable due to the nature of the traumatic event (sexual, physical assault, non-man-made-disasters, and death of a close relative). Nature and strength of traumatization are associated with specific clusters od ressources and symptoms (especially dissociation, hyperarousal, and psychotizism). Victims of sexual violence show lowest levels of ressources and highest symptom pressure, followed by victims of physical violence and both non-intentional traumagroups. Scientific and therapeutical utilizations are discussed within the limitations of a cross-sectional study.
