Evaluierung eines Impfstoffes zur Vorbeugung der Rhodococcus equi-Pneumonie beim Fohlen

The aim of this study was to investigate the efficacy of an attenuated live vaccine Equillis RhodE Batch 10E12 (Fa. MSD, Boxmeer, NL) in foals in preventing the pneumonia caused by Rhodococcus equi (R. equi) and ist innocuity. This study was held on a stud with warmblood horses in Germany, in which R. equi is endemic. It was designed as a prospective double blind study with a control group. A group of 30 foals was administered a rectal vaccine and the control group consisted of 30 foals and was administered a placebo. The rectal vaccination of the foals occurred on day 0, 1, 14 and 15 of the study. During the study which lasted 26 weeks, all foals were frequently examined clinically and the lungs were evaluated by sonography in order to detect signs of pneumonia or side effects of the vaccine. In addition blood samples were taken at seven time points during the study to get serum and to detect antibody titers with ELISA. Further seven blood samples were collected to get PBMCs from the foals, which were incubated with R. equi antigen and PMA to measure cytokines with Multiplex technology. The pneumonia was diagnosed in 23 of the 30 vaccinated foals. In the control group 27 of 30 foals developed pneumonia. In vaccinated foals pneumonia was diagnosed on day 98 ± 32 of the study and the control foals on day 89 ± 39. The disease lasted in both groups 24 days from diagnosis till complete recovery without treatment. There was also no difference in the severity of the disease between vaccinated and control foals. IgG titers against R. equi cell wall extract were measured during the study. In those characteristic curves the presence of maternal antibodies, immunological gap and indigenous production of IgG combined with rising infection were clearly apparent in all foals. However there was no difference in antibody titers between the two groups. In addition healthy and sick foals did not differ in antibody titers either. To identify the cellular immune response of the foals after vaccination, PBMC were stimulated with R. equi antigen and the generated cytokines were measured with multiplex technology. The results demonstrated no significant differences in IL-4, IFN- , IL-10 and IL-17 values between vaccinated and control foals. Furthermore there were no differences between the amount of cytokines after R. equi stimulation and negative control within the groups. Though the positive control operated with PMA stimulation showed a clear cytokine response by an activated, cellular immune system even in very young foals. The PBMCs of the foals developed after PMA stimulation considerably more IL4 than IFN- , independently of the vaccination. Especially the amounts of IL-10 and IL-17 clearly increased during the study. These results demonstrate that the vaccine tested here does neither activate the humoral nor the cellular immune system of the foals and therefore it induces no clinical effects. Despite promising preliminary trials with that attenuated live vaccine, the dosing and way of application such as the antigen effect of the vaccine have to be fundamentally verified. More over this study shows that PBMCs of young foals are able to generate cytokines. More research needs to be done to generate a protective immune response in young foals by specific stimulation. According to the present results the humoral immune system seems to play a minor role in controlling R. equi.

Das Ziel der vorliegenden Studie war es, die Schutzwirkung der attenuierten Lebendvakzine Equillis RhodE Batch 10E12 (Fa. MSD, Boxmeer, NL) an Fohlen gegen die Pneumonie durch Rhodococcus equi (R. equi) sowie ihre Verträglichkeit zu untersuchen. Die Studie wurde auf einem deutschen Warmblutgestüt mit endemischer Rhodokokkose durchgeführt. In der Impfgruppe wurden 30 Fohlen mit der Vakzine geimpft und in der Kontrollgruppe wurden 30 weitere Fohlen mit einem Placebopräparat behandelt. Zum Zeitpunkt der ersten Impfung waren die Fohlen in beiden Gruppen im Mittel 12 Tage alt. Die Impfung der Fohlen erfolgte an den Studientagen 0, 1, 14 und 15 und der Impfstoff wurde rektal verabreicht. Die Fohlen wurden im Studienzeitraum von 26 Wochen regelmäßig klinisch und ultrasonografisch auf mögliche Krankheitsanzeichen und unerwünschte Wirkungen untersucht. Darüber hinaus wurde ihnen an sieben Untersuchungszeitpunkten im Studienverlauf Blut zur Antikörpertiterbestimmung mittels ELISA entnommen. Außerdem wurden den Fohlen an sieben weiteren Zeitpunkten Blutproben zur Gewinnung von Blutleukozyten entnommen, welche mit R. equi-Antigen und PMA stimuliert wurden und anschließend zur Cytokinbestimmung mittels Multiplexassay untersucht wurden. Die Diagnose "Pneumonie" wurde bei 23 der 30 geimpften Fohlen gestellt. In der Kontrollgruppe erkrankten 27 von 30 Fohlen. Die Fohlen der Impfgruppe erkrankten im Mittel an Studientag 98,3 ± 32,2, die Kontrollfohlen erkrankten im Mittel an Studientag 89,3 ± 39,3. Die milde Erkrankungsform dauerte in beiden Gruppen vom Zeitpunkt der Diagnosestellung bis zur vollständigen spontanen Genesung 24,5 Tage im Median. Auch der Schweregrad der Erkrankung unterschied sich bei geimpften und nicht geimpften Fohlen nicht. Die unzureichenden Unterschiede in der klinischen Wirksamkeit der Vakzine spiegeln sich auch im Vergleich der Antikörpertiterverläufe unter beiden Fohlengruppen wieder. Gemessen wurden IgG-Titer gegen R. equi-Zellwandextrakt, die im Studienverlauf für Fohlen charakteristische Kurven bildeten, in denen maternale Antikörper, immunologische Lücke und vermehrte IgG-Eigenproduktion in Kombination mit steigendem Infektionsdruck klar ersichtlich waren. Jedoch unterschieden sich die Antikörpertiter zwischen geimpften und Kontrollfohlen nicht. Darüber hinaus waren auch zwischen den erkrankten und nicht erkrankten Fohlen keine offensichtlichen Unterschiede hinsichtlich der Antikörpertiter zu erkennen. Zur Bestimmung der zellulären Immunantwort der Fohlen auf den Impfstoff wurden Blutleukozyten mit R. equi-Antigen stimuliert und die gebildeten Cytokine im Multiplexassay gemessen. Die Konzentrationen an IL-4, IFN- , IL-10 und IL-17 unterschieden sich nicht zwischen den geimpften und den Kontrollfohlen. Überdies waren innerhalb der Gruppen keine Unterschiede der gebildeten Cytokinmengen nach R. equi-Stimulation und Negativkontrolle ersichtlich. Die Positivkontrolle mittels PMA-Stimulation zeigte jedoch, dass die Produktion von Cytokinen durch ein aktiviertes, zelluläres Immunsystem schon beim jungen Fohlen möglich ist. Die Blutleukozyten der Fohlen bildeten nach PMA-Stimulation deutlich mehr IL-4 als IFN- unabhängig von der Impfung. Im Verlauf der vier Monate war eine deutliche Erhöhung der an IL-10 und IL-17 darstellbar. Aus diesen Ergebnissen lässt sich ableiten, dass der getestete Impfstoff weder das humorale noch das zelluläre Immunsystem der Fohlen stimuliert. Dies erklärt, dass dieser Impfstoff auch keine schützende Wirkung aufweist. Trotz vielversprechender Vorversuche mit der attenuierten Lebendvakzine muss neben Dosierung und Applikationsart auch die antigene Wirkung des Impfstoffes überprüft werden. Darüber hinaus zeigt die Studie, dass Blutleukozyten von Fohlen schon früh in der Lage sind Cytokine zu bilden. Es bedarf weiterer Untersuchungen die Fohlen-Blutleukozyten durch gezielte Antigenstimulation dazu zu bringen, Cytokine für eine schützende Immunantwort zu produzieren.
