Analyse des Hygienestatus im Rettungsdienst in Vorpommern und Erarbeitung einer Modellhygieneordnung für diesen Bereich als Fragestellung der Community Medicine

  • Es gibt für den Rettungsdienst keine einheitlichen Regelungen bezüglich hygienischer Voraussetzungen und Organisation des Infektionsschutzes. Aus diesem Grund sollte in allen Rettungswachen Vorpommerns einschließlich der Patientenübergabe an das Krankenhaus der Hygienestandard analysiert werden, um auf dieser Grundlage Empfehlungen zu erarbeiten. Mit einem standardisierten Fragebogen (119 Fragen) wurden 42 Rettungswachen analysiert. Ergänzend wurde als Stichprobe eine hygienisch- mikrobiologische Überprüfung zur Ermittlung potentieller Infektionsquellen im RTW an folgenden Untersuchungsstellen durchgeführt: Handwaschbecken, Sauerstoffinsufflation, manuelles bzw automatisches Beatmen, Blutdruckmessen, Beatmungskoffer, Bedienknopf des Beatmungsgerätes, Luftkammerschiene, Vakuummatratze, Schublade für Sterilgut und Handschuhe in Jackentasche. Die Fragebögen wurden von allen Rettungswachen beantwortet. Unabhängig von der Trägerschaft und vom Leistungserbringer sind die Voraussetzungen für ein hygienisches Qualitätsmanagement im wesentlichen gegeben. Das betrifft den baulichen Zustand und die Ausstattung der Rettungswachen, die Lagerung von Medikamenten und Verbandmaterialien, die adäquate Instrumentenaufbereitung, die korrekte Hände- und Wäschehygiene (Vorhandensein von Desinfektionsspendern im RTW), die Existenz von stich- und bruchfesten Sammelboxen im RTW sowie die Verwendung geschlossener Blutentnahmesysteme. In Einzelfällen sind aber gravierende Mängel festgestellt worden. Hier sind folgende Aspekte zu nennen: mangelndes Problembewußtsein, inkomplette Immunisierung (Ausnahme Hepatitis-A und B-Impfung beim Personal), kein genereller Einsatz von Sterilfiltern für Beatmungsgeräte, unzureichendes Desinfektionsregime, kritischer Umgang mit der Sauerstoffbefeuchtung, kein generelles Tragen von Schutzhandschuhen, unvollständige Ausstattung mit Mund-Nasen-Schutz, Tb-Masken und Schutzbrillen, keine Aktualisierung von Hygieneplänen, unklare Verantwortlichkeiten (Qualitätssicherung, Fehlen benannter Hygienebeauftragte), mangelnde Entsorgungsrichtlinien und Wasserbehälterhygiene. Zu erwähnen sind weiterhin die ungenaue Information der Rettungsteams über den Notfallpatienten und die noch zu verbessernde Weiterbetreuung von Notfallpatienten in der Klinik. Bei den in den Rettungswagen nachgewiesenen Keimen handelte es sich überwiegend um Hautkeime (koagulasenegative Staphylokokken, S. epidermidis) Es wurden aber auch grampositive Stäbchenbakterien, P. aeruginosa, Enterokokken und Dermatophyten isoliert. Das zeigt, daß die Händehygiene und die Desinfektion und Reinigung im RTW intensiviert werden müssen. Das Wasser aus dem Vorratsbehälter des Handwaschbeckens stellte ein nicht tolerierbares Erregerreservoir dar.
  • There are no uniform rules for the rescue service regarding hygienic preconditions and organisation of protection against infections. Therefore, based on analyses of hygiene standards which should be carried out in all ambulance stations of Pommerania, some recommendations have to be worked out. This includes the stations responsible transmission of patients to the hospitals. Using a standardised questionnaire (119 questions), 42 ambulance stations were analysed. In addition, a hygienic-microbiological spot-check was carried out to find out prospective sources of infections in ambulance cars. For that purpose, the following examination points were checked: sink, oxygen input, manual or automatic artificial respiration, blood-pressure apparatus, artificial respiration equipment, control knob of the artificial respiration equipment, air chamber splints, vacuum mattress, drawer for sterile equipment and gloves in jacket pocket. The questionnaires were answered by all ambulance stations. On the whole, the preconditions for observance of hygienic quality are being fulfilled, irrespective of ownership and staff. This is true for the constructional state of the ambulance stations, storage of medicine and first-aid material, proper preparation of instruments, correct hygiene of hands and clothes (availability of disinfectant suppliers in the ambulances), availability of proof collecting boxes of ultimate strength in the ambulance car as well as use of closed bloodletting systems. In some case, however, big deficiencies had to be stated. Here, above all, following aspects have to be mentioned lack of ability to recognise problems, incomplete vaccination (exclusion vaccination of the staff against hepatitis A and B), no general use of sterile filters for respiration equipment, insufficient disinfection schemes, critical approach to oxygen moistening, no general wearing of protecting gloves, insufficient equipment of mouth-nose- protection, tuberculosis masks and safety goggles, lack of actualising hygiene schemes, unclear responsibilities (quality control, lacking of people responsible for hygiene), lack of disposal rules and tank hygiene. Further to mention are the inexact information regarding the background of the emergency patient and the continued care for emergency patient in the hospital which still have to be improved. Mainly proven in the ambulances were germs of the resident skin flora. But positive rodshaped bacilli such as P. aeruginosa, enterococci and dermatophytes were isolated, too. This shows, that hand hygiene and disinfecting as well as cleaning in the ambulances still have to be intensified. The water from the tank of the sink represented a reservoir of germs which can not be tolerated.
