Tierexperimentelle Untersuchung zum Einheilverhalten von Calciumphosphatzement modifiziert mit Chondroitinsulfat

  • Gegenstand: Das Ziel der vorliegenden Arbeit war die experimentelle Untersuchung eines mit verschiedenen Zusätzen modifizierten Calciumphosphatzementes an Minischweinen. Als Alternative zu autogenen Knochentransplantaten gewinnen Knochenersatzmaterialien zunehmend an Bedeutung. Der mit Chondroitinsulfat modifizierte Calciumphosphatzement soll für die Sanierung von Knochendefekten genutzt werden. Diese Arbeit untersucht die osteogene Potenz kleiner funktioneller Gruppen der extrazellulären Matrix wie Chondroitinsulfat. Material und Methode: Insgesamt wurden 40 Defekte an zehn adulten LEWE-Minischweinen im Unterkieferbereich gesetzt (4 Defekte pro Minischwein) und mit Knochenersatzmaterial gefüllt. Ein mit Eigenblut und autologer Spongiosa gefüllter Defekt wurde als Referenz genutzt. Die drei anderen Defekte wurden entweder mit Biozement D, mit Biozement D/Kollagen-I-Gemisch oder Biozement D/Kollagen-I/Chondoitinsulfat-Gemisch gefüllt. Alle drei Knochenersatzmaterialien basierten auf einem pastösen Hydroxylapatit-Zement-Gemisch. Die Versuchsauswertung erfolgte mittels histologischer Imageanalyse am unentkalkten Trenn-Dünnschliffpräparat. Alle zehn Tiere konnten nach komplikationsloser Einheilung nach drei Monaten einer histomorphometrischen Bewertung zugeführt werden. Der intraoperationem aushärtende Hydroxylapatit-Kollagen-I-Chondroitinsulfat-Zement zeigte osteokonduktive Materialeigenschaften. Die vom Defektrand ausgehende Resorption und knöcherne Substitution gestaltete sich außerordentlich rasch. Nach 3-monatiger Einheilzeit konnte bei Zweidrittel der Defektfläche ein knöchernes Remodeling nachgewiesen werden. Schlussfolgerungen: Der neu entwickelte Hydroxylapatit-Kollagen-I-Chondroitinsulfat-Zement eignet sich als Knochenersatzmaterial am Tiermodell. Diese Schlussfolgerung basiert sowohl auf seiner guten knöchernen Integration und Resorption als auch durch seine gute Applizierbarkeit intra operationem. Der Zusatz von Chondroitinsulfat und Kollagen Typ I zu Calciumphosphatzementen führt zu einer statistisch signifikanten Steigerung des Knochenumbaus und der Knochenneubildung. Die vorliegenden Versuche belegen, dass durch Zugabe von Komponenten der extrazellulären Matrix wie Chondroitinsulfat die osteogene Potenz von Knochenersatzstoffen weiter gesteigert werden kann.
  • Subject: The purpose of this thesis is the description of the animal experimental study of calcium phosphate cement modified with various additives on mini pigs. Bone substitute materials are increasingly important as an alternative to autogenous bone grafts. Calcium phosphate cement modified with chondroitin sulfate shall be used for restoration of bone defects. This study examines the osteogenic potency of small functional groups of the extracellular matrix such as chondroitin sulfate. Materials and methods: A total of 40 standardized defects (4 bone defects per each mini pig) were placed in the mandibles of ten adult LEWE-mini-pigs and remodeled with bone substitute material. One of the defects was filled with autologous blood and autologous spongiosa so it could be used as reference. The other three defects were either filled with Biocement D, with Biocement D/collagen-I mixture or Biocement D/collagen-I/chondroitin sulfate mixture. All three bone replacement materials based on a pasty hydroxyapatite cement mixture. The test evaluation was performed by image analysis on histological undecalcified thin section preparation. Three month later all ten animals were fed to a histomorphometric evaluation after healing process without any complications. Results: The intraoperationem hardened hydroxyapatite-collagen chondroitin-I-cement showed osteoconductive properties. Starting from the edge of the defect the bone resorption and substitution ran off extraordinarily quickly. After 3-month healing a remodeling of bone could be demonstrated in two-thirds of the osseous defect area. Conclusions: Taking everything into consideration the new developed hydroxyapatite-collagen-I chondroitin sulfate cement is suitable as a bone substitute material in animal models. This conclusion is based both on his good bone resorption and integration as well as its good applicability intra operationem. The addition of chondroitin sulfate and collagen type I to calcium phosphate cements leads to a statistically significant increase of bone remodeling and bone formation. The present experiments show that the of osteogenic potency of bone substitutes can be further increased by addition of components of the extra-cellular matrix like chondroitin sulfate.
