Kephalometrische Charakterisierung eines kieferorthopädischen Patientenkollektivs anhand multivariat-statistischer Analysen

  • Um das heuristische Potential multidimensionaler Analysen exemplarisch zu untersuchen, wird ein kieferorthopädisches Patienten¬kollektiv (n=259, Alter 15,5 ± 7 Jahre) anhand der Anfangs-Fernröntgendaten mit Hilfe multivariat-statistischer Verfahren charakterisiert. Beim 1. Ansatz werden sämtliche kephalometrische Parameter der modifizierten Bergen-Analyse faktorisiert und die Patienten anhand der Faktorscores mittels Cluster¬analyse in sieben Gruppen klassifiziert. Der 2. Ansatz umfasst Clusteranalysen von Patientengruppen, die a priori anhand ihrer sagittalen Basenrelation als eindeutig „neutral“, „distal“ oder „mesial“ klassifiziert werden. Jeweils zwei bis drei Subcluster mit typischen Merkmalskonfigurationen werden identifiziert. Beim 3. Ansatz werden anhand der „Harmonievariablen“ SNA, SNB, ML-NSL, NL-NSL und NSBa nach Hasund und Segner fünf Hauptcluster mit insgesamt elf Subclustern gebildet, die sich hinsichtlich des Gesichtstyps sowie der sagittalen und vertikalen Harmonie und des Ausmaßes der Gesamtharmonie unterscheiden. Mit Hilfe der schrittweisen Diskriminanzanalyse werden die ökonomischsten Parametersätze bestimmt, die eine möglichst gute Reklassifikation der Clusterzugehörigkeit erlauben. Zwischen den Clusterlösungen der drei multivariaten Ansätze sind signifikante wechselseitige Überlappungen festzustellen.
  • Aiming to exemplify the heuristic potential of multidimensional analyses, an orthodontic patient sample (n=259, age 15.5 ± 7 years) is characterized by pre-treatment lateral cephalograms using multivariate statistical procedures. The 1st approach includes factor analysis of the entire modified Bergen parameter set and factor score-based cluster analytic patient classification into seven groups. The 2nd approach comprises cluster analyses of patient groups preclassified to display neutral, distal or mesial sagittal jaw relations. Two or three subclusters with characteristic cephalometric patterns were identified respectively. The 3rd approach based on the „harmony“ parameters SNA, SNB, MLNSL, NLNSL and NSBa after Hasund and Segner yields five main clusters and eleven subclusters which vary for facial type, sagittal and vertical harmony and the degree of overall harmony. Stepwise discriminant analysis reveals the most parsimonious variable sets that allow the best possible reclassification of cluster memberships. The cluster solutions produced by the three approaches show significant mutual overlaps.
