Inhalative Kombinationspräparate durch Co-Präzipitation

In der vorliegenden Arbeit wird eine neue Methode zur Herstellung von Pulvern zur Inhalation beschrieben, die zwei Wirkstoffe beinhalten. Dabei handelt es sich um das langwirksame beta-2-Sympathomimetikum Salmeterolxinafoat und das Glucocorticoid Fluticasonpropionat. Beide Substanzen werden in der Asthmatherapie eingesetzt und finden Anwendung in der Behandlung bei einem Schweregrad der Stufe drei und vier. Neuere Untersuchungen belegen eine synergistische Wirkung, wenn beide Substanzen an derselben Zielstruktur in der Lunge zur Wirkung kommen. Dazu ist eine Codeposition beider Wirkstoffe im Respirationstrakt notwendig. Die bisher auf dem Markt befindlichen Kombinationspräparate versuchen dies über eine physikalische Mischung der beiden Substanzen zu realisieren. In dieser Arbeit wird eine Copräzipitation der beiden Substanzen zur Herstellung von Wirkstoffpartikeln verwendet, die zur Inhalation geeignet sind und beide Wirkstoffe in einem Partikel vereinigen. Dazu werden beide Substanzen im therapeutischen Verhältnis in einem organischen Lösemittel gelöst und anschließend in einem Mikromischer im Verhältnis 1 + 10 kontinuierlich mit Wasser als Nichtlösemittel gemischt, das verschiedene oberflächenaktive Additive enthalten kann. Die so hergestellte Dispersion wird anschließend durch Sprühtrocknung in ein Pulver überführt. Dieses kann dann beispielsweise in Kapseln abgefüllt und mithilfe eines Pulverinhalators inhaliert werden. Das Lösemittel, aus dem die Präzipitation erfolgt, beeinflusst den Habitus der Partikel und damit auch die aerodynamischen Eigenschaften der resultierenden Pulver. Eine Präzipitation aus methanolischer Lösung ergibt flache nadelförmige Partikel, die mit einer Feinpartikelfraktion von zirka 30% gute aerodynamische Eigenschaften aufweisen. Auch die Additivauswahl und deren Einsatzkonzentration haben einen Einfluss auf die resultierende Partikelgröße, da sie ein Partikelwachstum bei der Präzipitation behindern können. Hier hat vor allem das eingesetzte Polysorbat 80 einen guten Effekt bewiesen. Darüber hinaus beeinflussen die Additive auch das aerodynamische Verhalten der Pulver, da sie sich feinverteilt im Pulver zwischen den Wirkstoffpartikeln befinden können und so das Dispergiervermögen deutlich verbessern. Dies wird in der vorliegenden Arbeit für Hydroxypropylmethylcellulose (HPMC) gezeigt, welche fein verteilte sphärische Partikel im Produkt ergibt, die die Feinpartikelfraktion des Pulvers signifikant erhöhen. Die optimierte Formulierung mit 0,01% Polysorbat 80 und 0,05% HPMC im Nichtlösemittel ergibt inhalierbare Pulver mit einer Feinpartikelfraktion von mehr als 40% bei einem Wirkstoffgehalt von etwa 50%. Anders als bei den meisten Pulvern zur Inhalation kann so auf die Mischung mit einem grobdispersen Trägerstoff verzichtet werden. Betrachtet man die in-vitro Deposition im Next Generation Pharmaceutical Impactor, so ist eine gleichmäßige Deposition der beiden Wirkstoffe zu beobachten, welche auf eine gute Co-Deposition schließen lässt. Die Bestimmung der Einzelpartikelzusammensetzung mithilfe der Fluoreszenzmikroskopie sowie der Raman-Mikroskopie zeigt, dass die hergestellten Partikel beide Wirkstoffe enthalten. Dabei kann eine unterschiedliche Partikelmorphologie mit unterschiedlichen Verhältnissen der Wirkstoffe im Partikel in Zusammenhang gebracht werden. Durch das vorgestellte Verfahren lässt sich ein Kombinationspräparat zur Therapie des Asthmas herstellen, das die synergistische Wirkung der beiden Wirkstoffe Salmeterolxinafoat und Fluticasonpropionat optimal ausnutzen kann, da die Wirkstoffe durch die Formulierung bereits auf partikulärer Ebene kombiniert werden und dadurch an denselben Wirkort gelangen.

In this work, a novel method for the formulation of dry powders for inhalation containing two active ingredients is described. The active ingredients used are Salmeterol xinafoate, a long acting beta-2-agonist, and Fluticasone propionate, a corticosteroid, which are both utilised in the therapy of asthma with a severity of stage three and four. Recent studies suggest a synergistic effect of these substances when acting at the same target cell in the lung. For this, both substances have to be co-deposited at the same place in the respiratory tract after inhalation. The formulations of the combination products being currently on the market, exclusively work with physical mixtures of 2-agonist and corticosteroid to address co-deposition in the lung. In this work, co-precipitation as a novel formulation approach is utilised for the production of particles capable of being inhaled and containing both active ingredients. For the precipitation, both substances are dissolved in a therapeutic ratio in an organic solvent. This solution is then mixed with the non-solvent water containing surface active additives in a ratio of 1 + 10 with continuous mixing in a micromixer. The obtained dispersion is spraydried subsequently to obtain a dry powder capable of being filled in a capsule and being dispersed with the help of an inhalation device. It is shown, that the solvent used for the precipitation, influences the resulting particle shape. A precipitation from methanol results in elongated needle-like particles having a good particle fraction of about 30% after aerodynamic assessment with the Next Generation Pharmaceutical Impactor (NGI). Further on, the choice of excipient and its concentration influences the particle size of the precipitate and its aerodynamic behaviour. Within the developed additive combination of Polysorbate 80 and hydroxypropyl methylcellulose (HPMC), Polysorbate can alter particle size and minimise particle growth after precipitation while HPMC can influence the aerodynamic behaviour depending on the used concentration. An excess concentration of HPMC results in small excipient spheres being evenly distributed throughout the dry product and acting as dispersion modifier. With an optimised formulation containing 0.01% Polysorbate 80 and 0.05% HPMC in the non-solvent solution, powders with about 50% drug content and with a fine particle fraction of more than 40% can be produced. In contrast to most of the dry powder inhalation formulations, the use of a coarse carrier like lactose can be avoided with these formulations. The two drugs are distributed very evenly throughout the stages of the NGI indicating good co-deposition. A closer analysis of the single particle composition with fluorescence microscopy and Raman microscopy revealed that every particle contains Salmeterol xinafoate as well as Fluticasone propionate. The proportion varies with differences in particle morphology. With the method described, a combination product can be created, which utilises the synergistic effect of beta-2-agonist and corticosteroid in an advantageous way, as the substances are already combined on a particulate level by the formulation and with this delivers both drugs to the same site of action.
