The Polycystic Ovary Syndrome (PCOS) concerns approximately 6 to 10 percent of all women of childbearing age and it is one of the most common endocrinological diseases in this phase of life. This dissertation analyzed retrospectively the routine health care of patients with PCOS in a medical center of endocrinology. The certainty of diagnosis under conditions of medical practice and the effect of a six to twelve month treatment with Metformin were analyzed. In addition it was referred to the controversial discussion of an appropriate method for testing insulin resistance. Patients with PCOS (n=102) differed clearly in many pathognomonic parameters from patients with excluded PCOS (n=58). Patients with PCOS suffered more from hirsutism and ovulatory dysfunction, exhibit higher levels of free androgen index (FAI), LH-FSH-quotient, fasting insulin, total insulin excretion and were insulin resistant by the indices HOMA-IR and QUICKI. SHBG, acne and the autoimmune thyroiditis hashimoto were inappropriate parameters to distinguish both groups. The results have to be evaluated against the background of an age difference of 7 years between both groups as well as the use of the Rotterdam criteria to define the PCOS. Under the treatment with metformin 30 obese patients with PCOS achieved significant decreases in weight and body mass index (BMI) as well as a significant reduction of ovulatory dysfunction. FAI decreased only in combination with an oral contraceptive, but still with hyperandrogenemia. Patients treated with Metformin were less insulin resistant with significant lower levels of fasting insulin and HOMA-IR and a higher level of QUICKI (in terms of a decreasing insulin resistance). The possible influence of dietary measures and sport was not taken into account, so the open issue is if Metformin on its one could reduce hyperandrogenemia in the course of decreased insulin resistance. HOMA-IR, QUICKI and a 2 hour oral glucose tolerance test were appropriate methods to distinguish insulin resistant patients with PCOS from patients without PCOS und without insulin resistance. The diagnostic value of HOMA-IR can be assessed as high due to the age and BMI of the analyzed patients. In addition HOMA-IR was more appropriate than a 2 hour oral glucose tolerance test.

Das Polyzystische Ovarsyndrom (PCOS)
betrifft circa 6 bis 10 Prozent aller Frauen im gebärfähigen Alter
und ist eine der häufigsten endokrinologischen Erkrankungen in
dieser Lebensphase. Die vorliegende Dissertation untersuchte
retrospektiv die Routineversorgung von Patientinnen mit PCOS in
einer Praxis für Endokrinologie. Es wurden die Diagnosesicherheit
des PCOS unter Praxisbedingungen und der therapeutische Effekt von
Metformin unter einer sechs- bis zwölfmonatigen Therapie
analysiert. Zudem wurde zur kontroversen Diskussion eines
geeigneten diagnostischen Mittels einer Insulinresistenz Stellung
bezogen. PCOS-Patientinnen (n=102) unterschieden sich in vielen
krankheitstypischen Parametern eindeutig von Patientinnen mit
ausgeschlossenem PCOS (n=58). PCOS-Patientinnen litten häufiger an
Hirsutismus und Zyklusstörungen, hatten einen höheren freien
Androgenindex (FAI) und LH/FSH-Quotienten, ein höheres
Nüchterninsulin, eine höhere Insulingesamtausschüttung und waren
anhand der Indizes HOMA-IR und QUICKI insulinresistent. Zur
Differenzierung ungeeignete Paramater waren SHBG, Akne und die
Hashimoto-Thyreoiditis. Die Ergebnisse sind vor dem Hintergrund
eines Altersunterschiedes von 7 Jahren zwischen beiden Gruppen,
einer Hyperandrogenämie anhand des FAI in beiden Gruppen sowie der
Anwendung der Rotterdam-Kriterien zur Definition des PCOS zu
werten. Unter der Therapie mit Metformin konnten bei 30 adipösen
PCOS-Patientinnen signifikante Body-Mass-Index-/ und
Gewichtsabnahmen sowie eine signifikante Reduktion der
Zyklusstörungen erzielt werden. Der FAI sank lediglich in
Kombination mit einem Ovulationshemmer signifikant, wobei weiterhin
eine Hyperandrogenämie vorlag. Die Patientinnen waren unter
Metformintherapie weniger insulinresistent mit signifikant
niedrigerem Nüchterninsulin, niedrigerem HOMA-IR und höherem QUICKI
(im Sinne einer abnehmenden Insulinresistenz). Der mögliche
Einfluss von diätetischen Maßnahmen und Sport fand keine
Berücksichtigung, sodass die Frage offen verblieb, ob Metformin
allein im Zuge verbesserter Insulinresistenz zur klinisch
relevanten Reduktion einer Hyperandrogenämie beiträgt. Zur
Diagnostik einer Insulinresistenz waren HOMA-IR, QUICKI und ein
OGTT über zwei Stunden geeignet, insulinresistente
PCOS-Patientinnen von Patientinnen ohne PCOS und ohne
Insulinresistenz zu unterscheiden. Die diagnostische Aussagekraft
des HOMA-IR kann bei den untersuchten Patientinnen auf Grund des
Alters und des BMI als hoch eingeschätzt werden. Zudem war der
HOMA-IR zur Verlaufskontrolle der Metformintherapie besser geeignet
als ein OGTT über zwei Stunden.
