Background Diagnosis of paroxysmal atrial fibrillation can be challenging, but is highly relevant in patients presenting with sinus rhythm, no history of atrial fibrillation and acute cerebral ischemia. We aimed to evaluate whether more people were diagnosed with paroxysmal atrial fibrillation through prolonged and enhanced monitoring and whether clinical predictors exist. Methods Find-AFRANDOMISED was a prospective multicenter study which randomised 398 acute ischemic stroke patients ≥ 60 years to prolonged and enhanced Holter ECG or usual stroke unit care. The intervention group received Holter ECG for 10 days at baseline, 3 and 6 months and the control group received only the usual 24 hours of monitoring. Vital parameters, blood levels and patient history were taken at baseline. BNP levels were determined at baseline and at 3 months. Results The study showed that patients in the intervention group were 3.3 times more likely to be diagnosed with atrial fibrillation than patients in the control group. BNP was a predictor for atrial fibrillation in the intervention group but not in the control group, while also a predictor in the entire cohort. Conclusions A Holter-ECG is a relatively easy and not very complex diagnostic detection method of atrial fibrillation and it can reduce the percentage of cryptogenic stroke with only minor drawbacks. Furthermore, routine determination of BNP levels in patients with ischemic stroke should be closer evaluated.

Hintergrund Paroxysmales Vorhofflimmern zu detektieren ist eine Herausforderung, aber hochrelevant bei Schlaganfallpatienten im Sinusrhythmus und ohne Vorhofflimmern in der Anamnese. Unser Ziel war eine Antwort auf die Frage, ob sich durch ein verlängertes und verbessertes Monitoring die Detektionsrate von Vorhofflimmern erhöht und ob klinische Prädiktionsfaktoren existieren. Methoden Find-AFRANDOMISED war eine prospektive multizentrische Studie, welche 398 Patienten ≥ 60 Jahre mit akutem, ischämischem Schlaganfall in Gruppen mit verlängertem und verbessertem Monitoring des Herzrhythmus oder einem Standard-Monitoring randomisierte. Die Interventionsgruppe erhielt bei Baseline und nach 3 und 6 Monaten ein 10-Tage-Langzeit-EKG und die Kontrollgruppe ein Standard-Monitoring über 24 Stunden. Des Weiteren wurden Vitalparameter und Blutwerte bestimmt sowie eine Anamnese hinsichtlich Vorerkrankungen durchgeführt. BNP wurde bei Baseline und nach 3 Monaten bestimmt. Ergebnisse Die Studie zeigte, dass eine Diagnose von Vorhofflimmern in der Interventionsgruppe 3,3fach wahrscheinlicher war als in der Kontrollgruppe. BNP ist ein unabhängiger Prädiktor von Vorhofflimmern in der Interventionsgruppe, jedoch nicht in der Kontrollgruppe, wobei BNP auch ein unabhängiger Prädiktor von Vorhofflimmern in der Gesamtkohorte ist. Zusammenfassung Das Langzeit-EKG ist eine verhältnismäßig einfache und wenig aufwendige diagnostische Detektionsmöglichkeit von Vorhofflimmern und kann den Anteil kryptogener Insulte bei nur geringen Nachteilen reduzieren. Weiterhin sollte die routinemäßige BNP-Bestimmung bei Patienten bei Patienten mit zerebraler Ischämie in Erwägung gezogen werden.
