Abstract: Der Prozeß des Tumorwachstums und der Metastasierung ist eine komplexe Kaskade von Ereignissen, wobei der essentielle Schritt die Basalmembrandestruktion durch proteolytischen Abbau der Extrazellulärmatrix (ECM) ist. Verantwortlich für den ECM-Abbau sind die Matrixmetalloproteasen (MMP), deren proteolytische Aktivität durch spezifische Inhibitoren (Tissue inhibitor of matrixmetalloproteinases TIMP) reguliert wird. In dieser Arbeit wurde die mRNA- und Proteinexpression der Proteasen MMP-2 und MMP-9 und deren Inhibitoren TIMP-1 und TIMP-2 untersucht. In einem ersten Schritt wurden die Transkriptionslevel von MMP-2, MMP-9, TIMP-1 und TIMP-2 mittels RT-PCR in jeweils 5 malignen Schilddrüsengeweben ermittelt. Dabei wurden die Transkripte von MMP-2 in 68%, MMP-9 in 72%, TIMP-1 in 84% und von TIMP-2 in 88% der Tumoren nachgewiesen. Anschließend erfolgte die immunhistochemische Untersuchung nach der Avidin/Biotin Komplex-Methode in Gefrierschnitten von 80 malignen Schilddrüsengeweben: 10 follikuläre (FTC), 17 papilläre (PTC), 7 schlecht differenzierte (pdTC), 16 undifferenzierte Schilddrüsenkarzinome (UTC), 30 medulläre Schilddrüsenkarzinome (MTC) und 15 benigne Schilddrüsengewebe (Struma colloides et nodosa). In den malignen Geweben war eine signifikant erhöhte Expression sowohl von MMP-2 und MMP-9 als auch von TIMP-1 und TIMP-2 zu beobachten. MMP-2 war in den FTC, PTC und pdTC hoch exprimiert, während in den UTC und MTC nur in 56% bzw. 40% der Karzinome eine erhöhte Expression feststellbar war. MMP-9 zeigte in allen malignen Schilddrüsengeweben eine starke Expression. TIMP-1 konnte in allen Tumoren beobachtet werden. Ein ähnliches Ergebnis wies TIMP-2 auf, auch hier zeigte sich in allen Schilddrüsenneoplasien eine deutliche Expression. In den benignen Schilddrüsengeweben konnte keine oder nur eine geringe Expression der beiden Proteasen und auch der Inhibitoren festgestellt werden. In Bezug auf pTNM-Stadium war keine Korrelation mit dem Expressionsgrad von MMP-2, MMP-9, TIMP-1 und TIMP-2 feststellbar. Die Ergebnisse zeigen, daß die Proteasen und auch die Inhibitoren mit dem malignen Verhalten der untersuchten Schilddrüsenneoplasien assoziiert sind.
The matrix metalloproteinases (MMPs) belong to the family of zinc dependent endopeptidases are capable of degrading components of the extracellular matrix. In addition, activities of MMPs are regulated by endogenous proteins known as tissue inhibitors of matrix metalloproteinases (TIMP). The imbalance between gene expression of MMPs and TIMPs seems to be caused an increased metastatic and invasive potential of the tumor cells. Aim of the investigations were to study the expression of mRNA of MMP-2 and his inhibitor TIMP-2, MMP-9 and TIMP-1 in 5 follicular (FTC), 5 papillary (PTC), 5 undifferentiated thyroid carcinoma (UTC) as well as the immunoreactivity in 19 FTC, 17 PTC, 7 poorly differentiated (pdTC), 16 UTC and 30 medullar thyroid carcinomas (MTC) as well as 15 benign thyroid tissues (Struma colloides et nodosa). RT-PCR analysis revealed that the transcript of MMP-2 was found in 68%, MMP-9 in 72%, TIMP-1 in 84% and by TIMP-2 in 88% of the tumors. A significant increased Immunoreactivity of MMP-2, MMP-9, TIMP-1 and TIMP-2. MMP-2 was found in the FTC, PTC and pdTC. Increased immunoreactivity was only detectable in the 56% of the UTC and 40% of the MTC. MMP-9 showed a strong expression in all malignant thyroid tissue. TIMP-1 and TIMP-2 could be observed in all tumors. In benign thyroid tissues the expression of the both proteinases and their inhibitors are very weakly detected or could not be found. There was no correlation between the tumor grade and the expression of MMP-2, MMP-9, TIMP-1 and TIMP-2. The data showed, that the expression of proteinases and inhibitors is associated with the malignancy of the investigated thyroid carcinomas.
