Zusammenfassung Es wurde der Nutzen der Elektromyostimulationsbehandlung bei frühfunktioneller Rehabilitation nach vorderer Kreuzbandersatzplastik (Operation nach Brückner, ambulant) in einer prospektiven, randomisierten Studie untersucht. Die Kontrollgruppe (48 Patienten) durchlief ein frühfunktionelles Rehabilitationsprogramm, die Versuchsgruppe (44 Patienten) führte zusätzlich ab dem 3. postoperativen Tag 3x täglich 30 min über 4 Wochen die Elektrostimulation des M. quadrizeps durch. Nach Rückgabe der Geräte wurde die Behandlungszeit notiert, dabei ergab sich, daß 68% der Patienten weniger als die Hälfte der vorgeschriebenen Zeit stimuliert hatte. Die Nachuntersuchungen (Cybex-Krafttest, Fastex- Koordinationstest, Oberschenkelumfangsbestimmung, Lysholm-Score) ergaben für beide Gruppen vergleichbare, insgesamt als gut einzuschätzende, Ergebnisse. Da die Elektrostimulationsbehandlung keinen nachweisbaren Nutzen erbrachte, kann diese bei vegleichbarem Nachbehandlungsschema nicht generell empfohlen werden.
Summary A prospective randomized study examined the benefits of electromyostimulation treatment in early functional rehabilitation after plasty of the anterior cruciate ligament (surgery according to Brückner, in outpatients). The control group (48 patients) underwent an early exercise rehabilitation program while the experimental group (44 patients) additionally applied at home electrostimulation of M. quadriceps for 3 x 30 min a day from the third postoperative day for a period of 4 weeks. When returning the devices the treatment times were recorded with the result that 68% of the patients had stimulated the muscle for a period less than half of the prescribed time. The follow-up examinations (Cybex strength test, Fastex coordination test, thigh girth, Lysholm score) provided comparable results for both groups which altogether could be assessed as good. As electrostimulation treatment did not render any detectable benefit, it cannot be generally recommended for comparable follow-up treatment schemes.
