Häufig stellt eine Lebertransplantation (LTx) die Ultima Ratio vieler Lebererkrankungen im Endstadium dar. Jedoch ist die Organfunktion nach Transplantation in einer beträchtlichen Anzahl der Fälle eingeschränkt oder unzureichend. Dabei stellt der Ischämie-Reperfusionsschaden (IRS) nach wie vor eine relevante Komplikation dar. Die Situation wird dadurch verschlimmert, dass in Anbetracht des vorherrschenden Organmangels eine zunehmende Allokation von Organen minderer Organqualität, sogenannter marginaler Organe, stattfindet. Denn marginale Organe bergen ein größeres Risiko für die Entwicklung eines schweren IRS. Die Aufklärung des Pathomechanismus ist daher ein wichtiger Schritt zur Entwicklung therapeutischer Strategien. In diesem Sinne wird in dieser Dissertationsschrift der frühe hepatische IRS in Bezug auf die LTx sowohl in einem Tiermodell als auch im Rahmen einer klinischen Fall-Kontroll-Studie analysiert.
Im Tiermodell des partiellen warmen IRS dieser Dissertation zeigen sich in histologischen, molekularbiologischen und durchflusszytometrischen Analysen nach 90-minütiger Ischämie und 24 Stunden nach Reperfusion hauptsächlich unkonventionelle IFN-γ⁻NK1.1⁻CD27⁻γδTCR⁺CD3⁺-T-Zellen, sogenannte γδ-T-Zellen, neben CD4⁻CD8⁻CD3⁺-T-Zellen, sogenannten doppelt-negativen T-Zellen, als die Effektorzellen des IRS. Diese sind von dem Transkriptionsfaktor RORγt abhängig und vermitteln den IRS über einen IL-17A-bedingten Mechanismus. Dabei wirkt sich die therapeutische Antagonisierung von RORγt mittels Digoxin protektiv gegenüber dem hepatischen IRS aus.
In der klinischen Fall-Kontroll-Studie sind 46 Patienten eingeschlossen, von denen 63 % einen frühen IRS aufweisen, histopathologisch diagnostiziert anhand der beiden Leberbiopsien vor Reperfusion am Ende der Kaltpräparation und etwa 2 Stunden nach Reperfusion kurz vor dem abdominellen Verschluss. Von 21 dieser Patienten wurden die Proben immunhistochemisch und molekularbiologisch analysiert. Dabei weisen die Ergebnisse darauf hin, dass TBX-21⁻RORc⁺TCRγ⁺CD3⁺-T-Zellen beim humanen, hepatischen IRS eine wichtige Rolle spielen, als Pendant zu den RORγt-abhängigen γδ-T-Zellen im Tiermodell. Somit scheinen entgegen bisheriger Annahmen nicht CD4⁺-Th1- und CD4⁺-Th17-Zellen die Effektorzellen des IRS zu sein, sondern vielmehr γδ-T-Zellen.
Entsprechend den von Eurotransplant definierten Kriterien wurde bei etwa 54 % der 46 Patienten ein marginales Transplantat alloziert. Diesbezüglich entwickeln Transplantate von Spendern mit hohem BMI, über 30%iger hepatischer Steatose sowie hohem Serumnatrium signifikant häufiger einen IRS. Damit stellt der IRS vor dem Hintergrund des vorherrschenden Organmangels mit der Folge einer vermehrten Allokation marginaler Organe ein zentrales Problem dar. In Anlehnung daran versterben Empfänger mit marginalem Organ signifikant häufiger und weisen signifikant schlechtere Überlebensdaten auf im Gegensatz zu Empfängern mit nicht marginalem Transplantat. Interessanterweise weisen Empfänger mit marginalem Lebertransplantat bei zusätzlicher Entwicklung eines IRS deutlich schlechtere Überlebensraten auf als Patienten ohne IRS. Dies spiegelt sich auch in signifikant schlechteren laborchemischen Verläufen nach Transplantation wider. Demgegenüber spielt es innerhalb der Gruppe der Patienten mit nicht marginalem Transplantat für das Überleben kaum eine Rolle, ob ein IRS an- oder abwesend ist.
Somit wird anhand der Überlebensdaten und des Verlaufs nach Transplantation die klinische Relevanz des IRS bei der orthotopen LTx insbesondere im Hinblick auf den vorherrschenden Organmangel ersichtlich, wobei γδ-T-Zellen beim frühen hepatischen IRS eine zentrale Rolle einnehmen.

Often, liver transplantation (LTx) is the ultima ratio of many end-stage liver diseases. However, post-transplant organ function is limited or inadequate in a significant number of cases. Ischemia reperfusion injury (IRS) is still a relevant complication. Given the current shortage of organs, the situation is made worse by the fact that there is an increasing allocation of organs of inferior quality, so-called marginal organs. The reason for this is that marginal organs carry a greater risk of developing a severe IRS. The elucidation of the pathomechanism is therefore an important step in the development of therapeutic strategies. Accordingly, this dissertation analyzes the early hepatic IRS associated with LTx both in an animal model and in a clinical case-control study.
In this study of an animal model of partial warm IRS histological, molecular biological and flow cytometric analyses following 90 minutes of hepatic ischemia and 24 hours of reperfusion revealed mainly unconventional IFN-γ⁻NK1.1⁻CD27⁻γδTCR⁺CD3⁺ T cells, so-called γδ T cells, besides CD4⁻CD8⁻CD3⁺ T cells, so-called double-negative T cells, as the effector cells of IRS. These are dependent on the transcription factor RORγt and mediate the IRS via an IL-17A-related mechanism. In this regard, the therapeutic antagonization of RORγt via digoxin is protective against hepatic IRS.
In the clinical case-control study including 46 patients 63 % had an early IRS, histopathologically diagnosed by the two liver biopsies before reperfusion at the end of cold preparation and about 2 hours after reperfusion shortly before abdominal closure. Of 21 of these patients the samples were analyzed by immunohistochemical and molecular biological analyses. The results indicate that TBX-21⁻RORc⁺TCRγ⁺CD3⁺ T cells play an important role in human hepatic IRS, as a counterpart to the RORγt-dependent γδ T cells in the animal model. Thus, contrary to previous assumptions CD4⁺ Th1 and CD4⁺ Th17 cells do not seem to be the effector cells of the IRS, but rather γδ T cells.
According to the criteria defined by Eurotransplant, a marginal transplant was allocated in approximately 54 % of the 46 patients. In this regard, transplants from donors with high BMI, over 30 % hepatic steatosis and high serum sodium develop an IRS significantly more frequent. Thus, against the background of the predominant organ deficiency the IRS is a central problem with the consequence of an increased allocation of marginal organs. In line with this, recipients with a marginal organ die significantly more often and have a significantly worse outcome than recipients with a non-marginal transplant. Interestingly, recipients with a marginal liver transplant and the additional development of an IRS have significantly worse survival rates than patients without an IRS. This is also reflected in significantly worse laboratory-chemical follow-up examinations after transplantation. In contrast, within the group of patients with non-marginal graft it is of little concern for survival whether an IRS is present or absent.
Thus, on the basis of survival data and postoperative course the clinical relevance of IRS in orthotopic LTx becomes apparent especially with regard to the predominant organ deficiency, with γδ T cells playing a central role in early hepatic IRS.
