Abstract

Background and aims In intensive care patients, the beta-lactam antibiotics ceftazidime, meropenem and piperacillin/tazobactam are routinely used to treat life-threatening infections. Many of these patients suffer from severe organ dysfunctions and require additional support. Furthermore, adding to the physical impairments of critically ill patients, renal replacement therapies (RRT) and extracorporeal lung support (ELS) profoundly affect the pharmacokinetics of beta-lactam antibiotics. Therefore, the aim of the presented study was to investigate the need to implement therapeutic drug monitoring (TDM) for these widely used beta-lactam antibiotics in critically ill patients on RRT or ELS. Methods First a HPLC-UV quantification method for the beta-lactams ceftazidime, meropenem and piperacillin in plasma samples was developed and validated for clinical use. It was applied to measure concentrations of ceftazidime, meropenem and piperacillin in human plasma of critically ill patients with different RRT or ELS from the intensive care unit of the Department of Medicine 1 of the Universitätsklinikum Erlangen. For each patient blood samples were collected at predetermined times during the study period. Antibiotic concentrations were then individually analyzed and evaluated in relation to the minimal inhibitory concentration (MIC). The pharmacokinetic software application Pharkin 4 was used to simulate and compare drug concentration-time profiles based on the measured values and published population data. Results The developed HPLC method is reliable and reproducible with a mean process effectiveness of > 90%. Stability and robustness studies demonstrated stability of the samples and solutions even beyond the scheduled processing times and storage conditions. Within the 18-month study period plasma samples from 16 different patients on RTT and/or ELS were analyzed using the developed method. Each of the included patients received one of three beta-lactam antibiotics. The measured plasma levels met the target of 100 %ƒt > 1×MIC in most cases. However, the target 100 %ƒt > 4×MIC was not attained with the selected dosage in 69,6 % of the trough samples and 3,1 % of the peak samples. In addition, alarmingly high plasma concentrations have been measured during our investigation every once in a while. The usage of the pharmacokinetic program Pharkin 4 led to no reliable prediction of the actual plasma concentration-time profiles by using either population data or patient-specific measurements. 1 Conclusions The developed HPLC method is suitable for quantitative measurement of the three investigated beta-lactam antibiotics ceftazidime, meropenem and piperacillin in EDTA plasma samples from critically ill patients with various RRT and/or ELS. We demonstrated it being applicable in clinical practice. The presented study demonstrates a high interindividual and intraindividual variability of the plasma concentrations of the three investigated beta-lactam antibiotics when using standard doses in critically ill patients on RRT and/or ELS. Furthermore the pharmacokinetic application Pharkin 4 could not reliably predict the plasma concentrations in these patients. In cases of alterations in pharmacokinetics, TDM can ensure adequate drug concentrations by permitting dose adjustments during the course of therapy. The results of this work strongly support the demand for TDM in critically ill patients with RTT and/or ELS during treatment with the beta-lactam antibiotics ceftazidime, meropenem and piperacillin.

Abstract

Hintergrund und Ziele Die Betalaktam-Antibiotika Ceftazidim, Meropenem und Piperacillin/Tazobactam werden häufig bei Intensivpatienten mit lebensbedrohlichen Infektionskrankheiten eingesetzt. Nicht selten kommt es bei diesen Patienten zum Ausfall lebenswichtiger Organsysteme und der Notwendigkeit diese vorübergehend zu ersetzen. Zusätzlich zu den Änderungen, die durch die veränderten physiologischen Prozesse bei kritisch kranken Patienten verursacht werden, können die hierbei eingesetzten Organersatzverfahren (OEV) die Pharmakokinetik der Betalaktam-Antibiotika erheblich beeinflussen. Das Ziel der vorliegenden Arbeit war es daher zu prüfen, ob bei kritisch Kranken mit organersetzenden Verfahren die Notwendigkeit der Etablierung eines Therapeutischen Drug Monitorings (TDM) für die Betalaktam- Antibiotika Ceftazidim, Meropenem und Piperacillin besteht. Methoden In einem ersten Schritt erfolgte die Entwicklung und Validierung einer Methode zur quantitativen Analyse der Plasmakonzentrationen von Ceftazidim, Meropenem und Piperacillin (Hochleistungsflüssigkeitschromatographie, HPLC). Die entwickelte Methode diente als Grundlage zur Messung der drei Betalaktam-Antibiotika in EDTA- Plasma-Proben von kritisch Kranken mit verschiedenen OEV auf der Intensivstation der Medizinischen Klinik 1 des Universitätsklinikums Erlangen. Für jeden Patienten wurden im Beobachtungszeitraum zu zuvor festgelegten Zeitpunkten Proben gewonnen und analysiert. Die gemessenen Plasmakonzentrationen wurden anschließend im intraindividuellen Verlauf sowie in Bezug auf die MHK betrachtet. Ergänzend wurden, mit Hilfe des Pharmakokinetikprogramms Pharkin 4, anhand der gemessenen Werte sowie anhand publizierter Populationsdaten, Arzneimittelkonzentrations-Zeit-Verläufe simuliert und verglichen. Ergebnisse und Beobachtungen Die durchgeführte Methodenvalidierung konnte zeigen, dass die entwickelte HPLC- Methode mit einer mittleren Prozess-Effektivität von > 90 % ein zuverlässiges und reproduzierbares Verfahren darstellt. Verschiedene Stabilitäts- und Robustheitsuntersuchungen wiesen eine Stabilität der Proben und Lösungen sogar über die vorgesehenen Bearbeitungszeiten und Lagerbedingungen hinaus nach. Innerhalb des 18-monatigen Untersuchungszeitraums wurden mit Hilfe der entwickelten Methode EDTA-Plasma Proben von 16 verschiedenen Patienten mit einem oder mehreren OEV analysiert. Jeder der eingeschlossenen Patienten erhielt eines der drei Betalaktam-Antibiotika. Die gemessenen Plasmaspiegel erfüllten in den meisten Fällen die Zielvorgabe von 100 %ƒt > 1×MHK. Das Ziel 100 %ƒt > 4×MHK 3 wurde mit den gewählten Dosierungen jedoch bei 69,6 % der Talspiegel und 3,1 % der Spitzenspiegel nicht erreicht. Neben zu niedrigen Plasmaspiegeln wurden in unserer Untersuchung auch vereinzelt sehr hohen Konzentrationen gemessen. Mit dem Pharmakokinetikprogramm Pharkin 4 konnte sowohl unter dem Einsatz von Populationsdaten als auch mit patientenindividuellen Spiegelmessungen keine zuverlässige Vorhersage der tatsächlichen Plasmakonzentrations-Zeit-Verläufe getroffen werden. Schlussfolgerungen Die entwickelte Methode stellt ein, für die quantitative Messung der drei Betalaktam- Antibiotika Ceftazidim, Meropenem und Piperacillin in EDTA-Plasma-Proben von kritisch Kranken mit verschiedenen OEV, geeignetes und im klinischen Alltag einsetzbares Verfahren dar. In der vorliegenden Arbeit konnte eine hohe inter- und intraindividuelle Variabilität der Plasmakonzentrationen der drei untersuchten Betalaktam-Antibiotika unter Standarddosierungen bei Intensivpatienten mit OEV aufgezeigt werden. Auch das Pharmakokinetikprogramm Pharkin 4 konnte die Plasmakonzentrationen bei kritisch Kranken mit OEV nicht zuverlässig vorhersagen. Ein TDM hingegen ermöglicht, trotz im Therapieverlauf wechselnder Pharmakokinetik, das Erreichen gewünschter Plasmakonzentrationen zu prüfen um Unter- oder Überdosierungen zu erkennen und zu reduzieren. Die Ergebnisse der vorliegenden Arbeit unterstützen die Forderung nach einem TDM bei kritisch Kranken mit OEV unter der Therapie mit den Betalaktam-Antibiotika Ceftazidim, Meropenem und Piperacillin.
