Abstract

Clinical examination and laparoscopy cannot be replaced by imaging techniques when diagnosing the MRKH-syndrome 1) Background: There are the following means of diagnosis for the identification of the genital malformations in patients with Mayer-Rokitansky-Küster-Hauser-Syndrome (MRKH-syndrome): clinical examination, ultrasound imaging, magnetic-resonance-imaging (MRI) and laparoscopy. This study compared the different diagnostic means in order to best classify the genital malformations. 2) Methods: The VCUAM-classification was used for the assessment of the genital malformation. The following reference methods were settled for the individual organs: vagina – clinical examination, cervix, uterus and adnexa – laparoscopy, kidney – MRI. The study analysed the concordance of the different diagnostic means with the reference methods in 138 patients with MRKH-syndrome. 3) Results: The vagina’s reference method was compared with the MRI where a kappa of 0,74 was found, meaning that the MRI is a possible but not ideal method for the assessment of the vagina. Ultrasound and laparoscopy did not help to classify the vagina. It was not possible to assess the best classification of the cervix because the examination findings rarely mentioned the cervix separately. MRI was a very good means in terms of classifying the uterus (kappa = 0,93) but also ultrasound imaging showed a high rate of agreement (kappa = 0,83). None of the imaging methods was able to achieve a sufficient classification of the adnexa. Both MRI and ultrasound imaging were good methods to find renal malformations (kappa = 0,87) 4) Conclusion: For the assessment of the uterus all 3 diagnostic methods (clinical rectal examination, ultrasound and MR-imaging) had good results of finding the correct classification in comparison with the laparoscopic findings. Malformations of the kidney were sufficiently found with the ultrasound imaging. None of the imaging methods could correctly classify the adnexa and vagina in comparison to the reference methods.

Abstract

Klinische Untersuchung und MRT sind ausreichend zur Diagnostik des MRKH-Syn-droms und zur Fehlbildungsklassifikation nach VCUAM 1) Fragestellung Für die Beurteilung genitaler Fehlbildungen bei Patientinnen mit Mayer-Rokitansky-Küster-Hauser (MRKH) Syndrom stehen folgende Untersuchungsmethoden zur Verfügung: klinische Untersuchung, Sonographie, Magnet-Resonanz-Tomographie (MRT) und Laparoskopie. Ziel der Untersuchung war der Vergleich der diagnostischen Methoden hinsichtlich der exakten Beurteilung der vorliegenden Fehlbildung. 2) Material und Methoden Die Beurteilung der Fehlbildung erfolgte anhand der VCUAM-Klassifikation. Folgende Referenzmethoden wurden für die einzelnen Organe festgelegt: Vagina –klinische Untersuchung, Zervix, Uterus und Adnexe – Laparoskopie und für die Beurteilung der Niere und ableitenden Harnorgane das MRT. Anschließend wurden bei 138 MRKH-Patientinnen die diagnostischen Verfahren hinsichtlich ihrer Übereinstimmung mit der jeweiligen Referenzmethode verglichen. 3) Ergebnisse: Verglichen mit der Referenzmethode war die Beurteilung der Vagina mit Hilfe des MRT nur bedingt möglich (Kappa=0.74), mit Hilfe der Sonographie und der Laparoskopie waren keine Aussagen zur Vagina möglich. Aussagen zur Zervix wurde mit Hilfe der bildgebenden Untersuchungsmethoden und der klinischen Untersuchung zu selten gemacht, so dass hier kein Vergleich mit der Laparoskopie möglich war. Eine sehr gute Beurteilung der Fehlbildungen des Uterus war mit Hilfe des MRT möglich (Kappa=0.93), die Sonographie (Kappa=0.83) zeigte ebenfalls häufig Übereinstimmungen. Mit keiner der diagnostischen Methoden war im Vergleich zur Referenzmethode eine exakte Beurteilung der Adnexe möglich. Hinsichtlich Nierenfehlbildungen zeigte sich eine gute Übereinstimmung in MRT und Sonographie (Kappa=0.87). 4) Schlussfolgerung Zur Beurteilung des Fehlbildungsspektrums bei MRKH-Patientinnen sind vor allem klinische Untersuchung und MRT, teilweise auch die Sonographie ausreichend. Die Beurteilung der Vagina ist nur durch eine suffiziente klinische Untersuchung, die der Adnexe nur mit Hilfe der Laparoskopie zufriedenstellend möglich, die meist im Rahmen der Anlage einer Neovagina ohnehin erfolgt.
