  • Background : The HIV-infection is charaterized by a severe impairment of both the cellular and humoral immune response. This may also have effects on the avidity maturation occuring during the course of microbial infections. We investigated the kinetics of the anti-HIV antibody avidity and examined whether the development of anti-HIV antibody avidity is affected by the course of the disease, the CD4 cell count and the viral load. Furthermore the influence of highly active antiretroviral therapy (HAART) on the avidity maturation was studied. Background : The HIV-infection is charaterized by a severe impairment of both the cellular and humoral immune response. Methods : The anti-gp41-, anti-gp120- and anti-p55-IgG antibody avidity were measured in one to three year intervals for up to ten years (1991-2001)in sera of totally 39 HIV-infected patients. Based on the course of the HIV-infection the patients were classified in four groups : 1. Long term survivors (LTS; n=12); Patients with rapid progression of the HIV-infection before the HAART era (PROG; n=11); 3. HAART treated patients (HAART; n=13); 4. Patients in the stage of seroconversion (SKP; n=3). A solid phase enzyme immunoassay using 4 M urea as elution reagent was developed to measure the antibody avidity. Results : Irrespective of the clinical status all patients revealed high (> 60%) avidity values for anti-gp41-IgG. Merely one of the patients in seoconversion showed a significant increase of the anti-gp41-IgG avidity. The avidity values for anti-gp120-IgG and anti-p55-IgG ranged from 25-91 % and 14-106 % respectively. The comparison of the avidity indices between the three collectives LTS, PROG and HAART displayed no significant difference. The level of CD4 cell count and viral load had no measurable effect on the avidity index. The correlation of the avidity differences for the time period before HAART begin with those under HAART application exhibited no significance. Conclusions : Although some patients revealed significant changes of the antibody avidity during the course of the infection, no evidence could be found that the development of anti-HIV antibody avidity was affected by the course of the disease, the immunological status or HAART.show moreshow less
