Zahlreiche Medikamente weisen in ihrem Nebenwirkungsprofil das Risiko von Rhabdomyolysen oder Myalgien auf. Unter TherapieDie Maligne Hyperthermie (MH) ist eine metabolische Myopathie. Zahlreiche Medikamente weisen in ihrem Nebenwirkungsprofil das Risiko von Rhabdomyolysen oder Myalgien auf. Unter Therapie von Cerivastatin kam es in 52 Fällen zu tödlichen Verläufen, so dass das Medikament 2001 vom Markt genommen wurde. Wir untersuchten Maligne Hyperthermie suszeptible Schweinemuskeln im In-vitro-Kontrakturtest (IVCT) unter Inkubation mit Atorvastatin, Simvastatin, Levofloxacin, Ciprofloxacin und Gemfibrozil. Für diese Medikamente sind derartige unerwünschte Arzneimittelwirkungen beschrieben. Wir konnten dabei zeigen, dass der Maligne Hyperthermie suszeptible (MHS) Muskel anscheinend empfindlicher auf Atorvastatin, Simvastatin, Levofloxacin und Ciprofloxacin reagiert als der Maligne Hyperthermie negative (MHN). Das zeigte sich anhand der gegenüber den nicht suszeptiblen Muskeln signifikant höher ansteigenden Ruhespannung der MHS-Muskelpräparate im IVCT. Wir erklären dieses Ergebnis durch die beim MHS-Muskel labilere Ca2+-Homöostase. Gemfibrozil scheint hingegen auf die Ca2+-Homöostase keinen Einfluss zu haben, jedoch in hohen Konzentrationen myotoxisch zu wirken. Eine Kombination mit anderen Medikamenten, die durch die gleichen Enzymsysteme abgebaut werden, sollte daher nur mit Vorsicht erfolgen, um eine Kumulation im Organismus zu vermeiden.show moreshow less

A lot of medicaments show up with a risk of rhabdomyolysis or myalgies as adverse drug reactions. We analysed malignant hyperthermia susceptible porcine skelet muscles in the in-vitro-contracture-test (IVCT) under incubation with atorvastatin, simvastatin, levofloxacin, ciprofloxacin and gemfibrozil. For these drugs, the described adverse reactions are established. We could show, that the malignant hyperthermia susceptible muscle seems to react more sensitive to atorvastatin, simvastatin, levofloxacin and ciprofloxacin than the malignant hyperthermia negative (MHN) muscle. This was shown by the in comparision to MHN muscles significantly higher raising baseline of MHS muscles in IVCT. We explain this result by the more instable Ca2+-homeostasis of MHS muscle cells. Gemfibrozil seems to not influence Ca2+-homeostasis, but to have toxic effects in high concentrations. A combination with other drugs which are exhausted by the same enzymatic system should thus be only used with caution, to prevent a cumulation in organsim.show moreshow less
