Im orofazialen Bereich finden diese Wünsche Ausdruck durch die vermehrte Anwendung von Botox zur Faltenminimierung, durch Liftingoperationen, aber auch durch ästhetisch und funktionell hochwertigen Zahnersatz wie implantatgetragenen Suprakonstruktionen. Um die Erfolge dieser Disziplin verifizieren zu können wurden in einer longitudinal angelegten Studie von Juli 1988In unserer Gesellschaft werden Bedürfnisse und Ansprüche durch Medien und Internet beeinflusst. Dabei wachsen unsere Wünsche nach attraktivem Aussehen und ansprechender Ästhetik. Im orofazialen Bereich finden diese Wünsche Ausdruck durch die vermehrte Anwendung von Botox zur Faltenminimierung, durch Liftingoperationen, aber auch durch ästhetisch und funktionell hochwertigen Zahnersatz wie implantatgetragenen Suprakonstruktionen. Um die Erfolge dieser Disziplin verifizieren zu können wurden in einer longitudinal angelegten Studie von Juli 1988 bis Februar 1999 an der Klinik für Mund- Kiefer- und Plastische Gesichtschirurgie der Universität Würzburg 339 Patienten mit 1144 Bone-Lock®-Implantaten versorgt und nachuntersucht. Die Auswertung der, bei den Recalluntersuchungen gewonnenen Daten mittels Tabellen und Kaplan-Meier-Überlebenskurven ergab eine 10-Jahres-Gesamterfolgsrate von 78,7 %. Bezüglich der Indikationsstellung sind die besten Werte beim zahnlosen Patienten mit einer Erfolgsquote von 81,3 % nach 10 Jahren festzustellen. Es folgt die Schaltlücke mit 77,8 % Erfolg, das Freiende mit einer Quote von 62,5 % und die Einzelzahnversorgung mit einer Zehnjahreserfolgsquote von 40,2 %. Mit zunehmender Implantatlänge und ansteigendem Implantatdurchmesser fallen die Verlustraten im Beobachtungszeitraum geringer aus. Die Beurteilung aller periimplantären Parameter zeigt eine gute Weichgewebssituation mit zufrieden stellenden Indexwerten. Im Einzelnen waren dies eine Hygieneerhebung nach Mombelli, der Sulkusblutungsindex nach Mühlemann und Son, der Gingivaindex nach Silness und Löe und die Ermittlung der Taschensondierungstiefen sowie des Mobilitätsgrades manuell und mit dem Periotestgerät. Die hohen Erfolgsquoten dieses Implantatsystems, die teilweise über den ermittelten Haltbarkeitswerten für konventionellen prothetischen Zahnersatz liegen und der Komfort sowie die Stabilität der darauf getragenen prothetischen Versorgungen rechtfertigen die invasive Vorgehensweise zur Rehabilitation teilweise unbezahnter und völlig zahnloser Patienten mittels Implantation.show moreshow less

The evaluation of the results of this study, with tables and Kaplan-Meier-survival curves showed a 10-year-total success rate of 78,7 %. Regarding the indications, the best results (81,3 % succsess after 10 years) are found at toothless patients. Followed by class-III-partial-dentition with the second best result of 77,8 %. Then free end situation with a rate of 62,5 % and a single implant with 10-year-success-rate of 40,2 %. With increasing the length of the implant and growing diameter the deficit rates reduced in the obersvation period. The valuation of all periimplant measurands showed a very good gum situation with contending index values. In detail, there are hygiene index by Mombelli69, sulcus bleeding index by Mühlemann und Son70, gingival index by Silness und Löe93 and the determination of pocket probe, manuell movement testing or movement testing with periotest. The excellent sucess rates of this implant system, partially better then the durability rates of conventionally prothetic dentures, the comfort and the stability of the supraconstuction justifies this kind of invasive approcach for rehabilitation of teethless patients with implants.show moreshow less
