Ziel der Studie war, um den Zusammenhang zwischen NGAL und HO-1 und der Inzidenz von AKI nach Herzoperationen zu untersuchen. Patienten und Methode: In dieser prospektiven Kohortenstudie wurden 150 erwachsene herzchirurgische Patienten untersucht. HO-1- und NGAL-Messungen wurden mittels ELISA durchgeführt. In uni- und multivariaten Analysen wurde der Zusammenhang zwischen Biomarkern und AKI untersucht. Ergebnisse: Insgesamt 35 Patienten (23,5%) entwickelten AKI in den ersten 48 postoperativen Stunden. Die Konzentrationen von NGAL und HO-1 waren bei ICU-Ankunft im Vergleich zum Ausgangswert höher (p<0.001 für NGAL bzw. HO-1). Bei Patienten, die AKI entwickelten, wurde 24 Stunden nach der Operation ein zusätzlicher Anstieg der Serum-HO-1-Konzentration beobachtet, während bei Patienten, die später keine AKI entwickelten, die Serum-HO-1-Konzentrationen abnahmen (AKI: 12.75 [8.69-34-35] vs. keine AKI: 8.30 [4.13-15.60] ng/ml, p=0.031). In der multivariaten Analyse wurde eine unabhängige Beziehung der Serum-HO-1-Konzentration 24 Stunden nach der Operation mit der Entwicklung einer AKI innerhalb 48 Stunden postoperativ gefunden (p=0.043, OR 1.053, 95% CI: 1.002-1.007). Zusammenfassung: Die Kinetik der HO-1-Serumkonzentrationen nach einer Herzoperation und der Zusammenhang der HO-1-Konzentration mit der Entwicklung von AKI liefert neue Informationen über die potenzielle Nützlichkeit von HO-1 als Biomarker.show moreshow less

In univariate and multivariate analyses, the association between biomarkers and AKI was investigated. Results: In total, 35 patients (23,5%) developed AKI during the first 48 hours after surgery. NGAL and HO-1 levels were higher at ICU arrival as compared to baseline (P<0.001 for NGAL and HO-1). NGAL levels were significantly higher in patients who developed AKI within 48 hours (960 [614–1,262] pg/mL vs 600 [461–783] pg/mL, P<0.001), while HO-1 levels did not differ (P=0.88) at this time point. In patients developing AKI, an additional increase of serum HO-1 levels 24 hours after surgery was observed, whereas in patients who subsequently did not develop AKI, serum HO-1 levels decreased (AKI: 12.75 [8.69–34.35] ng/mL vs no AKI: 8.30 [4.13–15.60] ng/mL, P=0.031). We found an independent relationship of serum HO-1 levels 24 hours after surgery with the development of AKI (P=0.043, OR 1.053, 95% CI: 1.002–1.107). Conclusion: The association of the second HO-1 burst 24 hours after CBP might help to distinguish between the causality of AKI in patients undergoing CBP, thus helping to adapt patient stratification and management.show moreshow less
