Zusammenfassung
In Deutschland leben ca. 4,7 Mio. Menschen mit einer Krebsdiagnose, und Schätzungen legen nahe, dass etwa ein Sechstel dieser Gruppe einen Migrationshintergrund hat. Migrantinnen und Migranten weisen teils unterschiedliche, oftmals niedrigere Krebshäufigkeiten im Vergleich zur Gesamtbevölkerung auf, besonders gut ist dies für die Gruppe der Aussiedlerinnen und Aussiedler dokumentiert. Zur onkologischen Versorgung dieser Gruppe liegen einige gezielte Studien vor, die z. B. aus dem vermehrten Vorliegen fortgeschrittener Stadien bei Erstdiagnose für einzelne Krebsarten auf das Vorliegen von Zugangsbarrieren in manchen Fällen schließen lassen. Fachärzte werden insbesondere von älteren Menschen mit eigener Migrationserfahrung seltener konsultiert. Zu den vermeidbaren Gründen für unterschiedliches Nutzungsverhalten in der onkologischen Versorgung zählen Sprachbarrieren, Informationsmängel bezüglich des Gesundheitssystems und die Erwartung von Verständigungsproblemen mit medizinischem Fachpersonal hinsichtlich soziokultureller Besonderheiten. Studien mit Brustkrebspatientinnen sowie von Kolorektalkrebs Betroffenen zeigen aber insgesamt eine hohe Zufriedenheit mit der onkologischen Versorgung. Bei der Palliativversorgung gibt es Hinweise auf eine deutliche Unterrepräsentation bestimmter Migrantengruppen und auch hinsichtlich der Teilnahme an Selbsthilfegruppen wird von Kommunikations- und anderen Barrieren berichtet. Für eine diversitätssensible onkologische Versorgung erscheint es daher wichtig, insbesondere im Bereich Kommunikation und Information Barrieren zu verringern sowie Kultursensibilität beim Personal und Teilhabe der Patientinnen und Patienten an Entscheidungen zur Versorgungsgestaltung zu stärken.
Abstract
Approximately 4.7 million people with a cancer diagnosis live in Germany, and about one sixth of this group is estimated to have a migrant background. Migrants show in some cases differing, often lower cancer incidences compared to the general population, and this is particularly well documented for the group of migrants to Germany, referred to as Aussiedler (resettlers). Studies on the oncological care of this group hint a the existence of care access barriers in some instances, as concluded from the increased frequency of advanced stages at first diagnosis for some cancers. Specialist physicians are used less frequently, especially by older people with their own migration experience. Avoidable reasons for varying utilisation behaviour in oncological care include language barriers, lack of information about the health care system and the expectation of communication problems with medical professionals concerning specific sociocultural characteristics. However, studies among breast cancer and colorectal cancer patients show high overall satisfaction with oncological care. In palliative care, there are indications of clear underrepresentation of certain migrant groups, and communication and other barriers to participation in self-help groups have been reported. For diversity-sensitive oncological care, it therefore seems important to reduce barriers, especially in the area of communication and information, and to strengthen the cultural sensitivity of staff as well as the participation of patients in decisions on the organization of care.
