Zusammenfassung
Hintergrund
Nach einem plötzlichen Todesfall befinden sich die nächsten Angehörigen häufig durch den unerwarteten Verlust und seine Umstände in einer akuten Belastungssituation. Seit 2009 existiert ein Kooperationsverfahren zwischen dem Institut für Rechtsmedizin (IfR) sowie dem Institut und der Poliklinik für Medizinische Psychologie, beide am Universitätsklinikum Hamburg-Eppendorf (UKE), um besonders betreuungsbedürftigen Angehörigen kurzfristig die Möglichkeit einer Krisenintervention vermitteln zu können.
Ziel der Arbeit
Es werden die Ergebnisse aus dieser 10-jährigen Kooperation berichtet. Typische Konstellationen, in denen Angehörige mit der Bewältigung eines Todesfalls im näheren Umfeld überfordert waren und professioneller Unterstützung bedurften, werden aufgezeigt.
Material und Methoden
Ausgewertet wurden die Protokolle des Instituts für Rechtsmedizin sowie die Behandlungsakten des Instituts und der Poliklinik für Medizinische Psychologie. In die Analyse wurden Hinterbliebene von „rechtsmedizinisch“ untersuchten Verstorbenen, die eine Einzelberatung erhalten hatten, aufgenommen. Das Kollektiv umfasst 50 Hinterbliebene im Alter von 23 bis 80 Jahren, davon 15 Männer und 35 Frauen. Das Alter der Verstorbenen reichte von unter einem bis zum 86. Lebensjahr, 35 waren männlich und 15 weiblich. Annähernd 50 % der Hilfesuchenden benötigten einen bis 3 Kontakte; die längste Behandlung erstreckte sich über mehr als 100 Termine.
Ergebnisse
Besondere Betreuung benötigen Hinterbliebene, deren Angehöriger unter dramatischen oder als sinnlos erlebten Umständen in ihrer Anwesenheit ums Leben kamen, sowie Hinterbliebene, deren psychische Ressourcen bereits vor diesem Todesfall reduziert waren. Die bisher nur in Hamburg modellhaft existierende Kooperation hat sich für die Hinterbliebenen, die in engem zeitlichen Zusammenhang zum Todesfall psychologische Hilfe erhalten konnten, als gut funktionierend erwiesen. In der Hälfte der Fälle war bereits eine geringe Interventionsfrequenz hilfreich.
Schlussfolgerung
Ergebnisse der vorgestellten Analyse u. a. weisen auf einen höheren Hilfebedarf als den zurzeit angebotenen hin. Bei diesen Angehörigen handelt es sich um eine im universitären Kontext rechtsmedizinischer Abläufe in Deutschland zu wenig wahrgenommene Gruppe, für deren Versorgung weitere Ressourcen benötigt werden.
Abstract
Background
Facing the sudden and unexpected death of a close relative and the surrounding circumstances may cause an acute stress disorder in traumatized next of kin. A cooperation procedure between the Institute of Legal Medicine and the Outpatient Clinic for Medical Psychology, both of the University Medical Hamburg-Eppendorf (UKE), has been in existence since 2009 in order to be able at short notice to mediate the possibility of a crisis intervention for relatives who are particularly in need of care.
Objective
The results of this 10-year cooperation are reported. Typical constellations in which relatives who were overwhelmed by coping with the death of a close relative and were in need of professsional support are demonstrated.
Material and methods
The protocols of the Institute of Legal Medicine as well as the treatment files of the Institute and the Outpatient Clinic for Medical Psychology were evaluated. Of those deceased examined under medicolegal aspects, relatives who had received individual counselling were included in the analysis. The collective included 50 bereaved persons aged 23–80 years, of whom 15 were men and 35 women. The age of the deceased ranged from <1 year old to 86 years old, of whom 35 were male and 15 female. Approximately 50 % of those seeking support needed 1–3 sessions and the longest duration of treatment extended to more than 100 sessions.
Results
Bereaved persons whose relatives died under dramatic or apparently senseless circumstances in their presence as well as bereaved persons whose mental resources were already reduced before the death, are in need of special care. The model cooperation, which so far only exists in Hamburg, has been proven to function well for bereaved persons who could receive psychological assistance in close temporal association to a fatality. In half of the cases even a low number of interventions was helpful.
Conclusion
The results of the presented analysis indicate among other things that there is a greater need for help than that which is currently available. In the university context of medicolegal procedures in Germany, these relatives are a group that has received too little attention and further resources are needed for their care.
