Summary
The antidotal therapy of methanol intoxication rests upon three therapeutic principles: 1. Administration of alkali (THAM buffer or sodium bicarbonate) to correct the metabolic acidosis, 2. application of ethanol to inhibit methanol oxidation, and 3. the giving of high doses of folic acid to accellerate the elimination of formic acid. The combined use of all three of these measures offers the best results.
With the application of ethanol, an immediate effect is obtained. The production of formic acid is instantaneously interrupted by the inhibition of methanol oxidation. By means of simultaneous administration of folic acid, the ability of the organism to eliminate formic acid is increased serveral times. Alkali in the form of THAM buffer or sodium bicarbonate should also be utilized in cases where metabolic acidosis already exists, in order to normalize the extracellular and intracellular pH and thereby increase the degree of formic acid dissociation. This suppresses the toxic effect upon cellular metabolism which rests solely upon the presence of free formic acid.
Zusammenfassung
Die Antidottherapie der Methanolvergiftung basiert auf 3 Behandlungsprinzipien: 1. der Applikation alkalisierender Substanzen (Trispuffer und Natriumbicarbonat) zur BekÄmpfung der metabolischen Acidose, 2. der Zufuhr von Äthanol zur Hemmung der Methanoloxydation und 3. der Gabe von hohen FolsÄuredosen zur Beschleunigung der AmeisensÄureelimination. Die besten Resultate verspricht der kombinierte Einsatz aller drei Verfahren. Mit der Zufuhr von Äthanol wird ein Soforteffekt erreicht, indem durch Hemmung der Methanoloxydation die Produktion der AmeisensÄure unmittelbar unterbrochen wird. Durch gleichzeitige Gabe von FolsÄure wird das Eliminationsvermögen des Organismus für AmeisensÄure um ein Vielfaches gesteigert. Alkali in Form von Trispuffer oder Natriumbicarbonat sollte zusÄtzlich bei schon bestehender metabolischer Acidose zur Anwendung kommen, um durch Normalisierung des extra- und intracellulÄren pH den Dissoziationsgrad der AmeisensÄure zu erhöhen und damit die schÄdigende Wirkung auf den Zellstoffwechsel, die nur der freien SÄure eigen ist, zu unterdrücken.
