The research project Neighborhood Care aims to enhance the communication as well as the the common knowledge among caregivers and caretakers. This paper describes the user-centered design approach on the basis of a diary as a part of the Neighborhood Care online platform. It shows how to apply different Participatory Design methods. Beginning with interviews determining the needs of caretakers. Utilizing focus groups consisting of nursing experts and the later use of prototypes that were developed and tested with older adults. As a result, the simplification of two assessment tools have been translated into a self-care diary.
