Einsparpotenziale durch intravenös verabreichtes Dalbavancin bei MRSA-Infektionen mit langer Behandlungsdauer – ein gesundheitsökonomisches Modell mit deutschen DRG-Daten

Einführung: Bei einer Reihe von Infektionen (z.B. komplizierte Haut-Weichteilinfektionen (ABSSI) und tiefen Wundinfektionen) ist eine lange intravenöse Therapiedauer erforderlich. MRSA spielt hier als Erreger immer noch eine wichtige Rolle. Mit Dalbavancin steht eine Substanz zur Verfügung, die eine Einmalgabe von 1500mg i.v. erlaubt und eine Wirkdauer von 10–14 Tagen ermöglicht. Insbesondere wenn Oralisierung nicht möglich ist, stellt Dalbavancin eine klinische Alternative dar. Krankenhäuser in Deutschland werden nach DRG mittels einer Fallpauschale bezahlt und die Verweildauer ist ein kritischer Kostenfaktor. Deshalb sollte ein klinisch-ökonomisches Modell zeigen, ob mit Dalbavancin Einsparungen möglich sind.
Methoden: Zunächst wurden gemeinsam mit klinischen Experten Infektionsentitäten definiert, bei denen aufgrund des typischen Verlaufs ggf. eine frühe Entlassung nach Einmalgabe der Substanz möglich ist. Im nächsten Schritt wurden die Infektionsentitäten nach ICD-10 kodiert. Über eine Abfrage beim statistischen Bundesamt wurden die DRGs ermittelt, in denen diese Infektionen häufig vorkommen. Die Rate an MRSA wurde ebenfalls abgefragt. Aus einer Krankenhausbenchmarkingdatenbank wurden die Verweildauern von Patienten ermittelt. Aus der Differenz der Verweildauern von Fällen ohne und mit MRSA Infektion wurde eine mögliche Einsparung ermittelt und die Ergebnisse nach den Entitäten ABSSSI sowie Wundinfektionen ausgewertet.
Ergebnisse; Es wurden die in Tabelle 1 [Tab. 1] genannten Infektionsentitäten (mit Untergruppen) gebildet. Die DRG Analyse ergab eine Verweildauer von 25,8 vs. 16 Tagen (mit/ohne MRSA Infektion) in der ABSSI Gruppe sowie 21,8 vs. 16,3 Tagen in der Wundinfektionsgruppe, wie in Tabelle 2 [Tab. 2] dargestellt. Eine frühe Entlassung führt zu Einsparungen von EUR 467,42 in der ABSSI Gruppe aber zu höheren Kosten und somit einem Verlust von EUR 856,01. Über beide Entitäten hinweg werden EUR 272,35 eingespart, wie in Tabelle 3 [Tab. 3] dargestellt.
Diskussion: Die Ergebnisse zeigen nicht für alle Entitäten eine Einsparung. Eine so genannte break-even Rechnung ergab, dass für ABSSI min. 7,78 Tage und in der Wundinfektionsgruppe min. 8,75 Tage eingespart werden müssen. Zu diskutieren ist die Oralisierung als meist günstigere Alternative, die jedoch nur bei entsprechender Compliance angewendet werden kann.
Zusammenfassung: Die Einmalgabe von Dalbavancin i.v. zusammen mit einer früheren Entlassung von rund 8 Tagen kann insbesondere bei ABSSI kosteneffektiv sein. Eine sorgfältige Selektion der Patienten hinsichtlich der Compliance ist zu empfehlen.
