Electrical nanopulse-capsulotomy: the Zepto system

The advancements in cataract surgery are very much linked to the opening of the lens capsule, the capulorhexis or capsulotomy. One of the biggest breakthrough of cataract surgery was the development of the continuous curvilinear capsulorhexis by Gimble and Neuhann in the late 1980’s.

In the recent years the advancement of femtosecond laser technology had perfect circular laser cut capsulotomy by a femtosecond laser has been shown to be better in several aspects compared to the manual capsulorhexis.

However, the adaptation of femtosecondlaser technology is slow especially because of the substantial investment.

Alternatives have been developed by industry to create a perfect round capsular opening. The electrical nanopulsecapsulotomy by the Zepto system has been introduced to the cataract community.

Zepto comprises a disposable handpiece, a capsulotomy tip, and a power supply. The tip is silicone, and there’s a nitinol ring that allows for folding. The system provides round, accurately sized, and well-centered capsulotomies using highly focused multipulse low-energy discharge.

The device can be inserted through a 2.2 mm incision and placed onto the anterior capsule. It is possible to centrate the Zepto device in the visual axes and it is also possible to apply the technology even if the pupil is not fully dilated

Autopsy eye studies have shown that the strength of the capsulotomy is superior to femto second laser applied capsulotomy.

In the lecture the device application and the result of the femto capsulotomy will be displayed with video material.
