Purpose: The VICTUS Femtosecond laser platform (Bausch&Lomb/Technolas Perfect Vision) offers a variety of different application possibilities including cataract, refractive and therapeutic procedures with enhanced control and precision.

Method: The new and innovative laser refractive cataract surgery (LRCS) technique was used for anterior capsulotomy and lens fragmentation prior to phacoemulsification in cataract cases of complicated pre-existing conditions. These conditions included brown/black cataract, white/mature cataract, Pseudoexfoliation Syndrome (PEX), lentodonesis, zonular atrophy or amblyopia. The centration and precision of the capsulotomy in different cataract densities, application of different fragmentation patterns and visual outcomes were assessed. Furthermore, LRCS is compared with standard phacoemulsification in an ongoing, prospective clinical study.

Results: The LRCS technique has been safely performed under both general and local anaesthesia. Capsulotomies were well centred, precise and reliable and could be applied to all cataract grades. Even difficult cases were successfully treated with satisfactory visual outcomes. A number of fragmentation patterns have been assessed. The clinical study comparing LRCS and standard phacoemulsification is ongoing.

Conclusion: LRCS on the VICTUS platform is a promising new cataract surgery technique which can be safely and effectively used even in cases of complicated pre-existing conditions. Further evaluation of this new technology is required.
