Les "processus explicatifs" en thérapie centrée sur la personne chez des "thérapeutes" de différents niveaux d’expérience : Une analyse expérimentale des interventions et de leurs implications cognitives et affectives

English French The aim of this experimental study in the field of intervention in clinical psychology is to analyse how students and therapists in training in clientcentered therapy (CCT) intervene with clients et perceive and manage information with reference to their cogntive and affective componants, in the course of therapeutic micro-processes. The theoretical part adresses different aspects of information processing in a clinical perspective (for ex., Power & Dalgleish, 1997), and with regard to the client-centered therapy and also to new approaches to interventions, especially the « explicative process » according to Sachse (1992). The therapists’ cognitive and affective comprehension and representation, as well as a comparison between novice and experienced therapists are presented with different theoretical and empirical contributions. The empirical part provides a complex experimental paradigm which implies a standardized presentation of the statements of five clients in three stages in the course of therapeutic sequences filmed in real time, and illustrating different levels of explicative processing (according to Sachse). The objective is to evaluate the quality of the therapeutic response and various cognitive and affective implications. The research design combines intra-subjects factors (different levels and directions of the client’s sequences) and a between-subject factor : novice clinical psychology students, before and after a practical course in basic skill training in CCT, are compared with psychologists in advanced post-grad training in CCT. Results show an effect of learning (out and inner experience), and of practical experience on the quality of interventions (explicative process, empathy and unconditional regard) of both the students following basic skills training and the psychologists in advanced training. Moreover, the latter can follow the clients better during a deepening process and perceive more precisely the direction of the client’s process, without underevaluating their proposals. Psychologists tend to feel more implicated affectively than cognitively during the processes, compared to the students without practice. In this sense, the quality of the interventions seems positively related to cognitive implication and negatively related to affective implication by the students. The opposite can be observed for the psychologists in postgraduate training. The limits of this research paradigm – therapy process simulation based on real clinical material – are discussed with regard to the possibiliy of taking into account the therapeutic alliance. Benefits are mentionned with regard to the perspective of integrating such a paradigm as part of training in the different therapeutic approaches. Dans le cadre de l’intervention en psychologie clinique, cette étude expérimentale a pour but d’analyser comment des étudiants et des thérapeutes en formation centrée sur la personne (TCP) interviennent auprès des clients, et perçoivent et gèrent l’information transmise en référence aux composantes cognitives et affectives, lors de micro-processus thérapeutiques. La partie théorique aborde différents aspects du traitement de l’information dans une perspective clinique (par ex., Power & Dalgleish, 1997), ainsi que la thérapie centrée sur la personne et les nouvelles approches orientées vers des interventions, principalement le « processus explicatif » selon Sachse (1992). La compréhension et la représentation cognitive et affective des thérapeutes, ainsi qu’une comparaison entre thérapeutes novices et expérimentés sont présentées au regard de différentes contributions théoriques et empiriques. La partie empirique prévoit un paradigme expérimental sophistiqué impliquant la présentation standardisée d’énoncés de cinq clients en trois étapes lors de séquences thérapeutiques filmées en temps réel et illustrant différents niveaux de traitement explicatif (selon l’approche de Sachse), afin d’évaluer la qualité de la réponse thérapeutique et certaines implications cognitives et affectives. Le design de recherche combine des facteurs intrasujets (différents niveaux et directions des séquences présentées par les clients) et un facteur inter-groupe : des étudiants en psychologie clinique, novices, et avant et après un cours pratique en thérapie centrée sur la personne, sont comparés à des psychologues en formation post-grade en TCP. Les résultats montrent un effet de l’apprentissage (en dehors et au sein de l’expérience vécue), et de l’expérience pratique sur la qualité des interventions (processus explicatif, empathie et regard positif) formulées par les étudiants avec expérience pratique et les psychologues en formation. De plus, ces derniers suivent mieux les clients lors d’un processus approfondissant et perçoivent plus précisément la direction du processus chez les clients, tout en ne sous-évaluant pas leurs propres offres. Les psychologues en formation ont tendance à plutôt se sentir impliqués affectivement que cognitivement, lors des processus, comparés aux étudiants sans pratique. Dans ce sens, la qualité des interventions semble être liée positivement avec l’implication cognitive et négativement avec l’implication affective chez les étudiants, tandis que l’inverse s’observe chez les psychologues en formation. Les limites de ce paradigme de recherche – simulation d’un processus thérapeutique basé sur un matériel clinique réel – sont discutées notamment par rapport à l’impossibilité de prendre en compte l’alliance thérapeutique. Les avantages sont soulignés quant à la perspective d’élément de formation dans différentes approches thérapeutiques.
