Die Expression von ERG, CRISP3 und SPINK1 charakterisiert unterschiedliche molekulare Subgruppen von Prostatakarzinom. Die Abfolge der Aktivierung dieser 3 Proteine während der Tumorprogression ist im Einzelfall jedoch nicht vollkommen verstanden. Basierend auf der Hypothese, dass die Untersuchung unterschiedlicher Bereiche eines Tumors Rückschlüsse auf die Reihenfolge erlaubt, in der die analysierten molekularen Ereignisse auftreten, wurde in der vorliegenden Arbeit ein spezieller Heterogenitäts-Gewebemikroarray (TMA) auf die immunhistochemisch nachweisbare Expression von ERG, CRISP3 und SPINK1 untersucht. Der TMA bestand aus insgesamt 3.170 Tumorproben (TMA Spots), welche 10 unterschiedliche Tumorareale von 317 Tumoren repräsentierten. Zunächst wurden 3 konsekutive Schnitte des TMAs mit Antikörpern gegen ERG, SPINK1 und CRISP3 gefärbt. Für die Analyse der zeitlichen Abfolge der Expression dieser 3 Marker wurden 299 der 317 Tumoren ausgewählt, bei denen mindestens 5 TMA Spots für alle drei Marker auswertbar waren. Ein Tumor wurde für „positiv“ für den jeweiligen Marker erachtet, wenn mindestens ein TMA Spot eine erkennbare Immunfärbung zeigte. Demnach waren positiv für ERG n=167 (56%) Tumoren, für CRISP3 n=174 (58%) und für SPINK1 n=68 (23%) Tumoren. Der Anteil der Tumoren mit homogener, in allen Tumorspots positiver Expression jedes Biomarkers und deren Ko-Expressionsmuster wurden genutzt, um die chronologische Abfolge der Aktivierung der 3 Marker und mögliche molekulare Subgruppen zu identifizieren. Es fanden sich 119 (71%) Tumoren mit homogener ERG Positivität, 28 (16%) Tumoren mit homogener CRISP3 Positivität, aber nur 1 (2%) Karzinome mit homogener SPINK Positivität. Dies deutet auf eine relativ frühe (ERG), mittlere (CRISP3) und späte (SPINK1) Expression dieser Proteine während der Tumorprogression. Die CRISP3 Färbung war stark mit ERG-positiven Karzinomen assoziiert, während SPINK1 massiv mit ERG-negativen Karzinomen einherging: CRISP3 war in 86% der ERG-positiven und nur in 24% der ERG-negativen Tumore nachweisbar (p<0.0001) während sich SPINK in 36% der ERG-negativen und nur in 12% der ERG-positiven Tumore finden ließ (p<0.0001). Die CRISP3 Expression entstand üblicherweise erst nach der ERG-Fusionsbildung: 67% der 119 homogen ERG-positiver Tumoren hatten kleine, CRISP3-positive Areale, während eine fokale ERG Positivität nur in 11% der 28 homogen CRISP3-positiven Tumore gesehen wurde (p < 0.0001). Die Analyse der Entstehungssequenz in 248 Prostatakarzinomen ergab, dass 12% der Tumoren CRISP3 exprimierten ohne dass vorher eine ERG Aktivierung vorlag, und dass 11% der Tumoren erst nach der Tumorentstehung ERG-positiv wurden.

Zusammenfassend stellen die Ergebnisse dieser Studie eine detaillierte Beschreibung der zeitlichen Abfolge der molekularen Ereignisse in Bezug auf ERG, CRISP3 und SPINK1 in knapp 300 Prostatakarzinomen dar. Die Expression von ERG ist typischerweise ein frühes, aber nicht unbedingt initiales Ereignis, was meistens die Expression von CRISP3 nach sich zieht. SPINK1 wird fast exklusiv in ERG-negativen Tumoren gefunden. Es gibt jedoch auch seltene Ausnahmen von diesen Regeln, die in weniger als 5% der Tumoren auftreten: CRISP3 Expression ohne vorherige ERG Aktivierung und SPINK1 Expression in ERG-positiven Tumoren. Besonders die CRISP3 Expression ohne vorherige ERG Aktivierung stützt die Annahme, dass CRISP3 eine ERG-unabhängige Rolle in der Tumorprogression spielen kann. Der Heterogenitäts-TMA Ansatz ist ein wertvolles Werkzeug zur Klärung der Abfolge molekularer Veränderungen in individuellen Tumoren.

The expression of ERG, CRISP3 und SPINK1 characterizes different molecular subsets of prostate cancers. The chronological sequence of molecular events involving activation of these three proteins during the tumor progression in individual cancers is not fully understood. Based on the hypothesis that the examination of different tumor areas allows to draw conclusions about the sequence in which the analyzed molecular events occur, the present study developed a special heterogeneity tissue microarray (TMA) to examine the immunohistochemically detectable expression of ERG, CRISP3 and SPINK1. From each of the 317 prostates, 10 different tumor-containing tissue blocks were selected for the TMA construction, resulting in a total of 3.170 tissue spots. Antibodies specific for ERG, SPINK1 and CRISP3 were applied to visualize the staining in three consecutively freshly cut TMA sections. 299 cancers with at least 5 TMA spots interpretable for all three markers were included in this study to further analyze the chronology of molecular events connected to these three proteins. A tumor was considered “positive” for a given marker if at least one TMA spot showed immunohistochemically detectable staining. Therefore n=167 (56%) cancers had positive results for ERG, n=174 (58%) cancers for CRISP3 and n=68 (23%) cancers for SPINK1. We used the fraction of tumors with homogeneous positivity (i.e. 100% interpretable spots per tumor shows a positive result) of each biomarker and their co-expression patterns in order to estimate whether these markers occur early or late during tumor development and identify possible molecular subset of cancers. Homogeneous positivity was found in 119 (71%) ERG-positive tumors. The fraction of homogeneously positive cancers was 28 (16%) for CRISP3 and only 1 (2%) for SPINK1. These different fractions of tumors suggest that these three markers occur at early (ERG), intermediate (CRISP3) and late (SPINK1) phases of cancer development. Co-expression was most frequent for ERG and CRISP3, but rare for CRISP3 and SPINK1.
Positive CRISP3 staining was tightly linked to ERG-positive cancers, while SPINK1 expression was strongly associated with ERG-negative cancers: Staining of CRISP3 was found in 86% ERG-positive cancers but only 24% ERG-negative cancers (p<0.0001). In contrast, staining of SPINK1 was found in 36% ERG-negative cancers as compared to 12 % ERG-positive cancers (p<0.0001).
CRISP3 expression typically develops after ERG fusion has formed: 67% of 119 homogeneously ERG-positive cancers had small CRISP3-positive areas, while focal ERG positivity was only seen in 11% of 28 homogeneously CRISP3-positive cancers (p < 0.0001). Analysis of the chronological sequence of molecular alterations in 248 prostate carcinomas showed that 12% of the tumors expressed CRISP3 without prior ERG activation and that 11% of the tumors only became ERG-positive after the tumor had developed

In summary, the results of this study provide a detailed description of the chronological sequence of molecular events connected to ERG, CRISP3 and SPINK1 expression in about 300 prostate cancers. ERG fusion is typically an early and potentially but not necessarily cancer-initiating event in prostate cancers usually followed by CRISP3 expression. SPINK1 expression is found almost exclusively in ERG-negative tumors. However, there are also rare exceptions to these rules that occur in less than 5% of the tumors: CRISP3 expression without previous ERG activation and SPINK1 expression in ERG-positive tumors. Especially the observation that CRISP3 expression without previous ERG activation provides clinical support for the assumption that CRISP3 has an own and active role in cancer progression. The heterogeneity TMA approach is a powerful tool to describe the chronology of molecular events in individual cancers.
