Titel: Mikrosatelliteninstabilität beim Pankreaskarzinom: Eine Tissue Microarray Studie an 599 Tumoren

Praktisch alle Patient:innen mit Pankreaskarzinom benötigen im Laufe ihrer Erkrankung eine systemische Therapie. Eine neue vielversprechende Option ist dabei eine Therapie mit einem Immuncheckpoint-Inhibitor Pembrolizumab, die für Pankreaskarzinome mit Mikrosatelliteninstabilität (MSI) zugelassen ist. Ziel der vorliegenden Untersuchung war die Klärung der Häufigkeit der MSI beim Pankreaskarzinom und die Beantwortung der Frage, ob die MSI in betroffenen Tumoren homogen oder heterogen vorkommt. Darüber hinaus sollte die Beziehung zwischen MSI und der Dichte tumorinfiltrierender CD8+- Lymphozyten bestimmt werden. Für die Untersuchung wurde ein Tissue Microarray (TMA) verwendet, der aus 528 Adenokarzinomen des Pankreas, 60 Adenokarzinomen der Papilla Vateri, 7 Azinuszellkarzinomen des Pankreas und 4 anderen seltenen Pankreasmalignomen besteht. Die TMA-Schnitte wurden immunhistochemisch auf die Expression der Mismatch Reparatur (MMR)-Proteine MLH1, MSH2, MSH6 und PMS2 sowie CD8 zum Nachweis zytotoxischer Lymphozyten untersucht. Während die Expression der MMR-Proteine manuell durch einen Pathologen ausgewertet wurden, wurden die Dichte der CD8+-Lymphozyten mit einer digitalen Analyseprogramm (HALO™) automatisch vorgenommen. Die Untersuchung führte zur Identifizierung von insgesamt 6 Adenokarzinomen des Pankreas, die am TMA Anzeichen für das Vorliegen einer MSI aufwiesen. Die nachfolgende Validierung an Großschnitten bestätigte das Vorliegen eines MMR-Defektes immunhistochemisch in 4 der 6 Fälle. 3 dieser Fälle zeigten auch in der PCR-Untersuchung eine MSI (MSI-high). 1 der 4 immunhistochemisch MMR-defizienten Tumoren war auf der DNA-Ebene mikrosatellitenstabil. Alle 4 Fälle mit MMR-Defekten zeigten den Defekt in der gesamten Tumormasse (insgesamt 19 Blöcke wurden untersucht), sodass die Defekte sich in allen Tumoren als komplett homogen erwiesen. Der MMR-Defekt war signifikant mit einer hohen Lymphozytenmenge assoziiert. Die durchschnittliche Anzahl von CD8+-Lymphozyten in den 4 mikrosatelliteninstabilen Tumoren beträgt 685,178 ± 139,23 Zellen/mm2 und bei mikrosatellitenstabilen Tumoren 217,918 ± 13,01 Zellen/mm2 (P = 0,0009). Die Dichte der CD8+-Lymphozyten und die Intensität der MMR-Proteinexpression waren unabhängig voneinander und korrelierten auch nicht mit dem Tumorphänotyp (TNM-Stadien, Differenzierungsgrad und Resektionsstatus). Aus den Daten wird gefolgert, dass die MSI bei Adenokarzinome des Pankreas in 0,76 % der Fälle (4/528) vorkommt. Die signifikante Beziehung zu einer erhöhten Lymphozytenzahl spricht für einen funktionellen relevanten Effekt der MSI auf das Immunsystem beim Pankreaskarzinom. Dies passt zur Annahme, dass mikrosatelliteninstabile Pankreaskarzinome von einer Therapie mit Immuncheckpoint-Inhibitoren profitieren könnten.

Practically all patients with pancreatic cancer require systemic therapy during the course of their disease. A new promising option is a therapy with an immune checkpoint inhibitor pembrolizumab, which is approved for pancreatic cancers with microsatellite instability (MSI). The aim of this study was to clarify the frequency of MSI in pancreatic cancer and to answer the question of whether MSI occurs homogeneously or heterogeneously in the affected tumors. In addition, the relationship between MSI and the density of tumor-infiltrating CD8+-lymphocytes should be determined. A tissue microarray (TMA) consisting of 528 pancreatic adenocarcinomas, 60 adenocarcinomas of the papilla of Vater, 7 acinar cell carcinomas of the pancreas and 4 other rare pancreatic carcinomas was used for this study. The TMA sections were examined immunohistochemically for the expression of the mismatch repair (MMR)-proteins MLH1, MSH2, MSH6 and PMS2 as well as CD8 to detect cytotoxic lymphocytes. While the expression of the MMR proteins was evaluated manually by a pathologist, the density of the CD8+-lymphocytes was determined automatically using a digital analysis program (HALO™). The investigation led to the identification of a total of 6 adenocarcinomas of the pancreas, which showed signs of the presence of MSI on the TMA. Subsequent validation on large sections confirmed the presence of an MMR defect immunohistochemically in 4 of the 6 cases. 3 of these cases also showed an MSI (MSI-high) in the PCR examination. 1 of the 4 immunohistochemically MMR-deficient tumors was microsatellite stable at the DNA level. All 4 cases with MMR defects showed the defect in the entire tumor mass (a total of 19 blocks were examined), so the defects were found to be completely homogeneous in all tumors. The MMR defect was significantly associated with a high lymphocyte count. The mean number of CD8+-lymphocytes in the 4 tumors with MSI is 685.178 ± 139.23 cells/mm2 and in microsatellite stable tumors 217.918 ± 13.01 cells/mm2 (P = 0.0009). The density of CD8+-lymphocytes and the intensity of MMR-protein expression were independent of each other and also did not correlate with the tumor phenotype (TNM stages, grade of differentiation and resection status). From the data it is concluded that MSI occurs in 0.76 % of pancreatic adenocarcinomas (4/528). The significant relationship to an increased number of lymphocytes speaks for a functionally relevant effect of MSI on the immune system in pancreatic cancers. This fits with the assumption that pancreatic cancers with MSI could benefit from therapy with immune checkpoint inhibitors.
