Das Mammakarzinom ist der häufigste Tumor bei Frauen. Circa jede zehnte Frau er-krankt im Laufe ihres Lebens an dieser Neoplasie. Für die Planung der Therapie ist es von zentraler Bedeutung, die Aggressivität individueller Tumoren richtig einzuschätzen, denn aggressive, mit Nebenwirkungen behaftete, adjuvante Therapien können bei be-troffenen Patientinnen zu einer Lebensverlängerung führen. Die flowzytometrische Be-stimmung des DNA-Gehaltes wurde früher als möglicher Prognosefaktor beim Mammakarzinom diskutiert. Die meisten Untersuchungen erfolgten an verhältnismäßig kleinen Patientenkollektiven und lieferten widersprüchliche Ergebnisse. Angesichts er-mutigender Daten zur prognostischen Bedeutung der DNA-Ploidie in anderen Tumoren wurden in diesem Projekt 522 Mammakarzinom-Primärtumoren flowzytometrisch unter-sucht. Von 469 auswertbaren Befunden waren 52,7% diploid, 34,8% aneuploid und 12,6% tetraploid. Für die statistischen Untersuchungen wurden aneuploide und tetra-ploide als „nicht-diploide“ Fälle zusammengefasst. Die Zahl der nicht-diploiden variierte mit dem histologischen Tumortyp. Nicht-Diploidie war signifikant häufiger in 365 Tumo-ren vom nicht-speziellen Typ (52,3%) als bei 64 lobulären Karzinomen (29,7%, p=0,0007). Die nicht-Diploidie war auch signifikant mit dem Tumorstadium und dem Dif-ferenzierungsgrad assoziiert. In allen Tumoren waren 42,2% von 263 pT1, 54,8% von 155 pT2, 31,3% von 16 pT3 und 57,1% von 14 pT4 nicht-diploid (p=0,0375) bezie-hungsweise es waren 31,0% von 71 Grad 1, 47,8% von 270 Grad 2 und 55,6% von 126 Grad 3 Tumoren nicht-diploid (p=0,0035). Die gleichen statischen Beziehungen fanden sich im größten histologischen Subset der Tumoren vom nicht-speziellen Typ. Hier waren 44,8% von 212 pT1, 61,7% von 120 pT2, 57,1% von 7 pT3 und 63,6% von 11 pT4 nicht-diploid (p=0,0226) beziehungsweise es waren 33,3% von 45 Grad1, 52,3% von 214 Grad2 und 61,0% von 105 Grad3 Tumoren nicht-diploid (p=0,0076). Keine signifikante Beziehung fand sich weder in allen Tumoren noch im Unterkollektiv der Tumoren vom nicht-speziellen Typ mit dem Nodalstatus, dem HER2-Status, der Expression von Östrogen- und Progesteronrezeptoren, der „Triple-Negativität“ und mit dem Ki67-Labeling-Index. Die Untersuchung der prognostischen Relevanz der unter-suchten Parameter ergab eine hochgradige prognostische Signifikanz des pT-Status, was die Qualität der klinischen Daten validiert. Für den Ploidie-Status fand sich aber weder in allen Tumoren, noch im Subkollektiv der Tumoren vom nicht-speziellen Typ eine Beziehung zur Prognose. Aus den Daten ist zu schließen, dass der Bestimmung der DNA-Ploidie beim Mammakarzinom keine klinische Relevanz zukommt.

Breast cancer is the most common tumor in women. About one in ten women suffer from this neoplasm during their lifetime. For the planning of the therapy, it is of central importance to correctly assess the aggressiveness of individual tumors, because aggressive, adjuvant therapies, although with side effects, can lead to a prolongation of life in affected patients. The flow cytometric determination of DNA content was previously discussed as a possible prognostic factor in breast cancer. Most of the studies were done on relatively small groups of patients and gave contradictory results. In view of encouraging data on the prognostic significance of DNA ploidy in other tumors, 522 primary breast cancer tumors were examined flow cytometrically in this project. Of 469 evaluable findings, 52.7% were diploid, 34.8% aneuploid, and 12.6% tetraploid. For the statistical studies, aneuploid and tetraploid have been summarized as "non-diploid" cases. The number of non-diploid varied with the histological tumor type. Non-diploidy was significantly more common in 365 non-specific type tumors (52.3%) than in 64 lobular carcinomas (29.7%, p = 0.0007). Non-diploidy was also significantly associated with the degree of differentiation and tumor stage. In all tumors, 42.2% of 263 pT1, 54.8% of 155 pT2, 31.3% of 16 pT3 and 57.1% of 14 pT4 were non-diploid (p = 0.0375), respectively 31.0% of 71 grade 1, 47.8% of 270 grade 2 and 55.6% of 126 grade 3 tumors non-diploid (p = 0.0035). The same static relationships were found in the largest histological subset of non-specific type tumors. Here, 44.8% of 212 pT1, 61.7% of 120 pT2, 57.1% of 7 pT3 and 63.6% of 11 pT4 were non-diploid (p = 0.0226) and 33.3% of 45 grade 1 , 52.3% of 214 grade 2 and 61.0% of 105 grade 3 were non-diploid tumors (p = 0.0076). No significant relationship was found in either the tumors or the subset of non-specific tumors with nodal status, HER2 status, estrogen and progesterone receptor expression, triple negativity, and Ki67 labeling-index. Examination of the prognostic relevance of the investigated parameters revealed a high degree of prog-nostic significance of the pT-status, which validated the quality of the clinical data. For the ploidy status, however, there was no relation to the prognosis either in all tumors or in the subgroup of non-specific type tumors. From the data, it can be concluded that the determination of DNA ploidy in breast cancer is of no clinical relevance.
