Abstract: In der vorliegenden In-vitro-Studie wurde das Randschlussverhalten von Kompositfüllungen der Klasse-II in Abhängigkeit von der Härte des Kavitätenbodens untersucht. Die Untersuchung sollte die Frage beantworten, wie sich der Randschluss von adhäsiven Kompositfüllungen der Klasse II bei verringerter Dentinhärte, wie sie z.B. klinisch nach unvollständiger Entfernung von Karies im Kavitätenboden auftritt, verändern. Es wurden an 32 unteren extrahierten humanen Weisheitszähnen jeweils mesial und distal Kavitäten der Klasse-II präpariert. Mit einem Rosenbohrer wurde am okklusalen und approximalen Kavitätenboden 1mm tief und 3mm im Durchmesser zusätzlich exkaviert. Zur Simulation von Restkaries im Dentin wurden in die okklusalen und approximalen Zusatzkavitäten an je acht Zähnen einseitig Materialien verschiedener Härtegrade unter der Kompositfüllung eingebracht. Dies waren Cavit G (Vickershärte (VH)2), Ketac Bond (VH51), Durelon 0,2g (VH20) und Durelon 0,25g (VH30). Anschließend wurden alle Kavitäten im Etch-and-rinse-Verfahren mit Syntac classic und Tetric Evo Ceram in Inkrementtechnik gefüllt. Es folgte eine Untersuchung des Randschlusses direkt nach dem Legen der Füllungen, nach 5.000 Zyklen thermischer Wechselbelastung bei 5°C und 55°C und nach mechanischer Belastung im Kausimulator mit 50 N für 100.000 Zyklen. Die Bewertung des Randschlusses erfolgte in einem Lichtmikroskop bei 200facher Vergrößerung in 200μm-Abschnitten jeweils im Zahnschmelz und im Dentin mit Hilfe der Parameter „Randspalt vorhanden“ / „klinisch akzeptabel“ (kein Spalt). Die Ergebnisse wurden mittels gepaartem T-Test (p < 0,05) statistisch ausgewertet. Die Bewertung des Randschlusses unmittelbar nach dem Legen der Füllungen ergab sowohl im Schmelz als auch im Dentin bei allen Versuchsgruppen keine signifikanten Unterschiede für den Parameter „klinisch akzeptabel“ gegenüber den Kontrollgruppen. Nach der vollständigen thermo-mechanischen Belastung lag bei den Füllungen mit Ketac Bond-Unterlage der Anteil spaltfreien Randes im Dentin bei 89,8% und bei den Kontrollfüllungen bei 96,1%. Der Unterschied war statistisch signifikant (p = 0,034). In den Versuchsgruppen mit Cavit, Durelon 0,2g und Durelon 0,25g gab es nur noch 69,5 – 79,5% klinisch akzeptable Füllungsränder im Dentin, während in den Kontrollgruppen noch 91,6% – 95,7% der Füllungsränder akzeptabel waren. Diese Unterschiede waren alle statistisch signifikant (Cavit p=0,011, Durelon 0,2g p=0,034, Durelon 0,25g p=0,005). Als Schlüsselergebnis dieser Untersuchung ist festzustellen, dass die Randdichte von adhäsiv befestigten Kompositfüllungen der Klasse II sich bei reduzierter Härte des Kavitätenbodens nach thermo-mechanischer Belastung verringert. Obwohl es sich hier um einen In-vitro-Modellversuch handelte, muss ein Einfluss von kariös erweichtem Dentin am Kavitätenboden auf den Randschluss von Kompositfüllungen auch für die klinische Situation in Betracht gezogen werden.
This in-vitro-study examined the marginal integrity of Class II composite fillings with soft cavity floors of the dentine. It shall answer the question, how much the stability and marginal integrity of adhesive fixed Class II composite fillings at reduced dentine hardness will change after an incomplete removal of caries in the cavity floor. The Class II cavities of 32 extracted lower human wisdom teeth have been dissected mesial and distal and excavated with the bud burr occlusal and approximal, 1mm deep and 3mm in the diameter. In order to simulate remaining caries, different cementum bases of variant hardness grade have been inserted to the mesial cavities of eight teeth occlusal and approximal. These were Cavit G (Vickers hardness (VH)2), Ketac Bond (VH51), Durelon 0,2g (VH20) and Durelon 0,25g (VH30). Then all the cavities were filled using the Etch-and-rinse process with Syntac classic and Tetric Evo Ceram Increment technique. An examination of the border seal followed directly after the placement of fillings, after 5.000 cycles of thermic variation in stress at 5°C and 55°C and after automatic stress in the cyclic loading with 50 N for 100.000 cycles. All this has been effected under optical microscope with 200fold magnification in 200μm-segments respectively in the tooth enamel and the dentine. The results have been analysed statistically by use of paired T-Test (p < 0.05). The examination after the placement of fillings concerning the parameter „gap free margin“ revealed no significant differences for all test groups towards the control groups, and that as well in the enamel as in the dentine. After the complete thermic and automatic stress, the rate of gap free margins in the dentine was at 89,8% regarding the fillings on a Ketac-base, and 96,1% regarding the control groups. The difference was clinically significant (p = 0,034). Within the test groups using Cavit, Durelon 0,2g and Durelon 0,25g, only 69,5% to 79,5% of the filling borders had no gap in the dentine, while the control groups revealed 91,6% to 95,7% gap free margins. These differences were significant (Cavit p=0,011, Durelon 0,2g p=0,034, Durelon 0,25g p=0,005). In conclusion it should be noted that the marginal integrity of adhesively fixed Class II composite fillings is decreasing when leaving soft areas in the cavity floor area. Although this has been a model test, more examinations concerning the leaving of carious dentine within the dentine will be necessary.
