Abstract: Hintergrund: Seit 2012 empfiehlt die Kommission für Krankenhaushygiene und Infektionsprävention (KRINKO) für auf der neonatologischen Intensivstation (NICU) behandelte Frühgeborene mit einem Geburtsgewicht unter 1.500 g (Very low birth weight infants, VLBWI) ein wöchentliches Kolonisationsscreening (KoS) auf multiresistente (MRE) und besonders pathogene Erreger. Unter MRE werden subsummiert: multiresistente gramnegative Erreger (MRGN), Methicillin-resistenter S. aureus (MRSA) und Vancomycin-resistente Enterokokken (VRE). Die beiden Ziele des KoS sind: 1. Prävention der horizontalen Transmission und nachfolgenden nosokomialen Infektion (NI); 2. Wirksame empirische Antibiotikatherapie (ABT) von NI bei VLBWI mit vorbestehender MRE-Kolonisation. Es wird diskutiert, ob das KoS dem Konzept des Antibiotic Stewardship (ABS) dadurch entgegenwirkt, dass vermehrt Reserveantibiotika wie Carbapeneme als empirische ABT eingesetzt werden, wenn bei durch MRGN kolonisierten VLBWI eine NI auftritt. Carbapeneme sollten ausschließlich bei NI durch MRGN oder bei NI mit Organversagen eingesetzt werden. Fragestellungen: Auswirkung des KoS auf die ABT, insbesondere den Einsatz von Reserveantibiotika; Auswirkung des KoS auf mikrobiologische Diagnostik und ABT der Late onset sepsis (LOS); Prävalenz von MRE-Kolonisation und durch MRE verursachter NI; Risikofaktoren der MRGN-Kolonisation; Einfluss der ABT auf den MRGN-Erstnachweis; Indikationen einer Carbapenem-Therapie; Standard operation procedures (SOPs) und Drug Monitoring; Identifikation für das Qualitätsmanagement (QM) relevanter Prozesse auch zur Vorbereitung der Einführung eines ABS-Programms. Material und Methodik: Retrospektive Kohortenanalyse aller 3 Jahre vor (2009-2011) bis 3 Jahre nach Einführung des KoS (2012-2014) auf der NICU und der neonatologischen Intermediate Care (IMC), Klinik für Allgemeine Pädiatrie und Neonatologie des Universitätsklinikums des Saarlandes, behandelten VLBWI. Untersuchung der ABT in Form der Length of Therapy (LoT), Days of Therapy (DoT) in Abhängigkeit von NI, Device-Anwendung und Diagnostik bei Verdacht auf LOS (Blutkultur, Spitzen zentraler (Central line, CL) Gefäßkatheter). Ergebnisse: Verglichen mit der Kohorte vor Einführung (n=191) zeigte sich nach Einführung des KoS (n=201) eine signifikante Abnahme von LoT (p<0,001) und DoT (p<0,001). Dies war durch die signifikante Abnahme von LoT (p<0,001) und DoT (p<0,001) in der Geburtsgewichtsklasse 1.000-1.499 g bedingt; in dieser waren die DoT für Carbapeneme (p=0,009) und Teicoplanin (p=0,040) signifikant niedriger, was durch eine signifikante seltenere LOS (p=0,025) verursacht wurde. In den beiden anderen Geburtsgewichtsklassen zeigte sich kein signifikanter Unterschied der LoT (p=1,000 für VLBWI <500 g, p=0,754 für VLBWI von 500-999 g). Die DoT für Carbapeneme war in der Gesamtkohorte nach 21 Einführung des KoS nicht signifikant unterschiedlich (p=0,341). Signifikante Unterschiede in der ABT nach Einführung des Kos fanden sich in der Gesamtkohorte nur als höhere DoT für Cefuroxim (p=0,003) und niedrigere DoT für Ciprofloxacin (p=0,046). Die Kohorte nach Einführung des KoS wies ein signifikant niedrigeres medianes Geburtsgewicht (1.060 g vs. 1.120 g, p=0,046) und eine signifikant längere Verweildauer (68 Tage vs. 61 Tage, p=0,015) auf. Mit Ausnahme der Geburtsgewichtsklasse 1.000-1.499 g bestand kein signifikanter Unterschied (p=0,948) in der LOS-Prävalenz (n=77 vs. n=95). Die Anwendungsrate der Devices CL (p=0,079), periphere Venenverweilkanüle [PVK (p=0,209)] und ihre Assoziation zu einer Sepsis (CL: p=0,611, PVK: p=0,071) war nicht signifikant unterschiedlich. Die Praxis der Blutkultur- und CL-Diagnostik bei LOS unterschied sich nach Einführung des KoS nicht signifikant von derjenigen vor Einführung: Eine Blutkulturdiagnostik wurde bei 61 % vs. 59 % (p=0,916) der Episoden, eine CL-Diagnostik bei 93,5 vs. 94,7 % (p=0,916) der Episoden durchgeführt. In der Gesamtkohorte (n=392) wurde bei 91 VLBWI ein MRGN nachgewiesen; hiervon 55 nach Einführung des KoS (2MRGN in 83/91 (91,2 %) der Fälle). Eine LOS durch zuvor im KoS nachgewiesene MRE wurde nicht beobachtet; zwei Blutstrominfektionen (BSI) durch MRGN waren zugleich der Erstnachweis. Im KoS MRGN-positive VLBWI (n=55) wiesen verglichen mit MRGN-negativen (n=146) eine signifikant längere Verweildauer auf NICU (p=0,002) und IMC (p=0,015) auf; Geburtsgewicht (p=0,147) und Gestationsalter (p=0,197) waren nicht signifikant verschieden. Die Anwendungsrate von CL (p=0,001) und PVK (p=0,001) war bei MRGN-positiven VLBWI signifikant höher, die LOS-Prävalenz (p=0,067) hingegen nicht signifikant verschieden. Die LoT war signifikant länger (p=0,001), die DoT (p=0,001) signifikant höher; der Einsatz von Ampicillin/Sulbactam (DoT, p=0,002) und Gentamicin (DoT, p=0,021) war signifikant mit dem Nachweis eines MRGN assoziiert. Ein MRGN wurde im KoS im Median am 43. Lebenstag (Interquartilsabstand 26-86, Min. 1, Max. 133) nachgewiesen; je länger die LoT (p=0,020) und je höher die DoT für Carbapeneme (p=0,023), desto signifikant später war der Erstnachweis. Eine ABT mit Carbapenemen (n=122) war in der Gesamtpopulation signifikant häufiger, je niedriger Geburtsgewicht (p<0,001) und Gestationsalter (p<0,001) und je höher die Anwendungsrate von CL (p<0,001) und PVK (p<0,001) waren. Carbapeneme wurden bei MRGN-positiven VLBWI nicht signifikant häufiger (p=0,167) eingesetzt; ihr Einsatz war auch zumeist nicht durch MRGN-Positivität im KoS begründet. SOPs zu Blutkulturdiagnostik und ABT der LOS lagen nicht vor; Vancomycin und Gentamicin wurden ohne ein Drug Monitoring eingesetzt. Die Gesamtkohorte weist abgesehen von der 22 seltenen Nekrotisierenden Enterokolitis (n=10; nur Verdachtsfälle) keine strukturellen Besonderheiten auf. Schlussfolgerungen: Die Einführung des KoS führte weder zu einer Verbrauchszunahme von Reserveantibiotika noch zu einer Änderung der Blutkultur- und CL-Diagnostik der LOS. Mit Ausnahme von VLBWI der Geburtsgewichtsklasse 1.000-1.499, welche nach Einführung des KoS eine Abnahme der LOS-Prävalenz unbekannter Ursache aufwiesen, war der ausbleibende Verbrauchsanstieg in den anderen Geburtsgewichtsklassen und der Gesamtkohorte nicht durch eine niedrigere Rate von NI bedingt. Da der Erstnachweis eines MRGN im KoS überwiegend erst nach der kritischen Phase der Intensivtherapie erfolgte, hatte dieser offensichtlich keine Auswirkung mehr auf den Einsatz von Carbapenemen. Verweildauer und LoT, insbesondere DoT für Ampicillin/Sulbactam und DoT für Gentamicin, wurden als Risikofaktoren der MRGN-Kolonisation identifiziert. Eine Carbapenem-Therapie führte zu einem signifikant späteren MRGN-Erstnachweis; dies könnte darauf hindeuten, dass MRGN im KoS solange nicht nachgewiesen wurden, wie eine gegen MRGN wirksame Carbapenem-Therapie erfolgte. Es sollte durch weitere Studien untersucht werden, ob die Sensitivität des KoS während einer gegen die zu untersuchenden Pathogene wirksamen ABT reduziert ist. Als QM-relevante Prozesse wurde das Fehlen von SOPs und Drug Monitoring sowie das Missverhältnis von Blutkultur- zu CL-Diagnostik identifiziert. Die Einführung eines ABS-Programms erfordert Investitionen in Strukturen (patientenbezogene elektronische Verordnung der ABT, SOPs) und Personal (infektiologische Audits).
Background: Since 2012, the Commission for Hospital Hygiene and Infection Prevention (KRINKO, Germany) recommends a weekly colonization screening (KoS) for multidrug-resistant (MRE) and highly pathogenic bacteria in very low birth weight (< 1.500 g) infants (VLBWI). The term “MRE” refers to multidrug-resistant gram-negative pathogens (MRGN), methicillin-resistant S. aureus (MRSA) and vancomycin-resistant enterococci (VRE). The KoS serves two purposes: 1. Prevention of horizontal transmission and subsequent nosocomial infection (NI); 2. Effective empirical antibiotic therapy (ABT) of NI in VLWBI with a history of MRGN colonization. There is an ongoing debate whether MRGN colonization increases the use of carbapenems as a first-line treatment when NI is suspected; such an increase would be in stark contrast to the concept of antibiotic stewardship (ABS). Carbapenems should be reserved for the empiric treatment of NI caused by MRGN or NI complicated by organ failure. Objectives: To assess the impact of KoS on ABT, in particular on the use of third-line antibiotics; to determine its impact on microbiologic diagnostics and on ABT of late onset sepsis (LOS); to evaluate the prevalence of MRE colonization and NI caused by MRE; to determine risk factors of MRGN colonization; to assess factors influencing the first detection of MRGN; to determine the use of carbapenems; to evaluate standard operation procedures (SOPs) and drug monitoring; to identify relevant issues for quality management (QM) prior to the implementation of an ABS program. Methods: Retrospective cohort analysis of all VLBWI treated 3 years before (2009-2011) and 3 years after the introduction of the KoS (2012-2014); Site: Tertiary NICU and neonatal intermediate care (IMC) Unit, Department of General Pediatrics and Neonatology, University Hospital of Saarland; assessment of ABT [Length of Therapy (LoT) and Days of Therapy (DoT)] with regard to occurrence of NI, use of intravascular devices and microbiologic diagnostics (blood culture, tips of central vascular catheters, CL) when LOS was suspected. Results: Compared to the cohort prior to introduction of the KoS (n=191), a significant decrease in LoT (p<0.001) and DoT (p<0.001) was seen thereafter (n=201). This was due to a significant decrease in LoT (p<0.001) and DoT (p<0.001) in the birth weight category of 1,000-1,499 g; in this weight category, the DoT for carbapenems (p=0.009) and teicoplanin (p=0.040) were significantly lower, which was caused by a significantly rarer occurrence of LOS (p=0.025). In the other two birth weight categories, no significant difference in LoT (p=1,000 for the category of <500 g, p=0.754 for the category of 500-999 g) was seen. In the total cohort, the DoT for carbapenems was not significantly different (p=0.341) after introduction of the KoS; significant differences in ABT were only noted for a higher DoT for cefuroxime (p=0.003) and a lower DoT for ciprofloxacin (p=0.046). 24 After the introduction of the KoS, a significantly lower median birth weight (1,060 g vs. 1,120 g, p=0.046) and a significantly longer in-hospital stay (68 days vs. 61 days, p=0.015) were seen. Apart from the birth weight category of 1,000-1,499 g, there was no significant difference (p=0.948) in LOS prevalence (n=77 vs. n=95). The use of CLs (p=0.079) and peripheral intravenous cannulas [PVK (p=0.209)] and their association with sepsis (CL: p=0.611, PVK: p=0.071) were not significantly different. Microbiologic evaluation of suspected LOS (blood culture and CL tips) was not significantly affected by the KoS (p=0.916): Blood culture was performed in 61,0 % vs. 59,0 % (p=0,916) of episodes, CL diagnostics in 93,5 vs. 94,7 % (p=0,916). In the total study cohort (n=392), MRGN was detected in 91 VLBWI (55 by KoS) with 83/91 (91.2 %) being 2MRGN. There was no case of a LOS caused by MRE previously detected by KoS; two bloodstream infections (BSI) caused by MRGN were not detected by KoS. MRGN-positive VLBWI (n=55, after introduction of the KoS) spent significantly more time in NICU (p=0.002) and IMC (p=0.015) compared to MRGN-negative ones (n=146); birth weight (p=0.147) and gestational age (p=0.197) did not significantly differ between the two groups. The use of CL (p=0.001) and PVK (p=0.001) was significantly higher in MRGN-positive VLBWI, but the LOS prevalence (p=0.067) was not significantly different. LoT (p=0.001) was significantly longer, DoT (p=0.001) significantly higher in MRGN-positive VLBWI; the use of ampicillin/sulbactam (DoT, p=0.002) and gentamicin (DoT, p=0.021) was significantly associated with the detection of MRGN. Median time of detection of MRGN by KoS was 43 days of life (interquartile range 26-86, min. 1, max. 133): the longer the LoT (p=0.020) and the higher the DoT for carbapenems (p=0.023), the later the first detection by KoS. Carbapenems were frequently used in the total study cohort (n=122) with a significant association with lower birth weight (p<0.001), lower gestational age (p<0.001), and more frequent use of CL (p<0.001) and PVK (p<0.001). In contrast, carbapenems were not used significantly more frequently in MRGN-positive VLBWI (p=0.167); their use was also mostly not due to MRGN positivity in the KoS. SOPs for blood culture diagnostics and for ABT of LOS were not available; vancomycin and gentamicin were used without drug monitoring. The total study cohort showed no unusual demographic characteristics - aside from the rareness of necrotizing enterocolitis (n=10; only suspected cases, no surgery required). Conclusions: Neither an increase in the of use of third-line antibiotics, nor a modification of microbiologic diagnostics of suspected LOS (blood culture and CL tips) were seen after introduction of the KoS. Aside from the birth weight category of 1,000-1,499 demonstrating a lower LOS prevalence of unknown cause, changes in the rate of NI did not account for the use of third-line antibiotics in the other categories of birth weight and the total cohort. 25 Since first detection of MRGN by the KoS mainly occurred after the critical phase of intensive care therapy, it did not influence the use of carbapenems. In-hospital stay, LoT, DoT for ampicillin/sulbactam and DoT for gentamicin were identifiable risk factors for MRGN colonization. Carbapenems significantly delayed the first detection of MRGN; this suggests that MRGN are not detected by KoS while Carbapenems – effective against MRGN – are used. Further studies are required to verify that the sensitivity of KoS is reduced by ABT if ABT is effective against the pathogens targeted by KoS. Lack of SOPs and drug monitoring as well as a mismatch between blood culture diagnostics and CL diagnostics were QM-relevant processes. The introduction of an ABS program requires investments in adequate infrastructure (e.g., patient-based electronic prescription of ABTs, SOPs) and human resources (e.g., infectiological audits).
