Abstract: Ziel der Studie: Für ausgewählte Patienten mit einem oligometastasierten nicht-kleinzelligen Bronchialkarzinom haben radikale Therapieansätze ein kuratives Potential. Es ist jedoch unklar, ob synchrone und metachrone Manifestierung der oligometastasierten Erkrankung gleichermaßen für die ablative Behandlung geeignet sind und was dieser Faktor für die klinische Entscheidungsfindung bedeutet. Aus diesem Grund wurde eine vergleichende Analyse der Überlebenszeiten zweier Patientengruppen mit oligometastasiertem nichtkleinzelligen Bronchialkarzinom (synchron vs. metachron) durchgeführt. Material und Methoden: Fünfundsiebzig Patienten mit radikal behandeltem oligometastasierten nicht-kleinzelligen Bronchialkarzinom wurden identifiziert, davon hatten 39 eine synchrone und 36 eine metachrone Manifestation der Oligometastasierung. Für Patienten mit synchroner Metastasierung war die zusätzliche kurativ intendierte Behandlung der lokoregionalen Erkrankung (Chirurgie, Radiochemotherapie oder definitive stereotaktische Radiotherapie) Voraussetzung. Alle Patienten mit metachroner Metastasierung hatten eine anhaltende Remission des Primärtumors. Die ablative Therapie der kompletten oligometastatischen Ausdehnung bestand entweder aus kompletter Resektion mit oder ohne adjuvante Radiotherapie oder definitiver Radiotherapie mit einer biologisch effektiven Dosis von 60 Gy. Die Endpunkte der Analyse waren das mediane Gesamtüberleben und das progressionsfreie Überleben. Ergebnisse: Von 75 Patienten hatten 57 Patienten eine solitäre Metastase, nur 7 Patienten hatten Metastasen in ≥ 2 Organen und 66 Patienten hatten einen Karnofsky-Index von 80 % oder 90 %. Die mediane Nachbeobachtungszeit betrug 54 Monate (95 % CI 28-81), das mediane Gesamtüberleben 21.8 Monate (16.1-27.6) und das mediane progressionsfreie Überleben 13.7 Monate ((9.7-17.6). In der univariaten Cox-Regressions-Analyse war kein Faktor signifikant mit dem Gesamtüberleben assoziiert. Mit dem progressionsfreien Überleben waren der ‘metastatische Befall von ≥ 2 Organen vs. 1 Organ’ (hazard ratio 0.43, 0.23-0.83, p=0.012) und der ‘Karnofsky-Index 90 % vs. 70 % - 80 %’ (hazard ratio 4.32, 1.73-10.89, p=0.02) als signifikante Faktoren in der multivariaten Analyse assoziiert. Zwischen den beiden Gruppen mit synchroner (n = 39) und metachroner (n = 36) Oligometastasierung gab es keinen Unterschied bezüglich des medianen Gesamtüberlebens und des progressionsfreien Überlebens. Beide Kohortengruppen waren gut balanciert, bis auf den Karnofsky-Index, dieser war in der Gruppe mit der synchronen Metastasierung signifikant besser. Schlussfolgerung: Die radikale Behandlung des oligometastasierten nicht-kleinzelligen Bronchialkarzinoms war bei Patienten mit einem guten Karnofsky-Index mit akzeptablen Langzeitüberlebensraten assoziiert und für synchrone und metachrone Manifestationen gleichermaßen effektiv.
Background: Radical treatment for oligometastatic non-small-cell lung cancer has a curative potential for selected patients. It is yet unclear whether synchronous and metachronous manifestations of oligometastatic disease are equally suitable for ablative treatments and what this factor means for clinical decision making. Therefore, a comparative survival analysis of two groups of patients with oligometastatic non-small-cell lung cancer (synchronous vs. metachronous) was performed. Methods and Materials: Seventy-five patients with radically treated oligometastatic nonsmall- cell lung cancer were identified, of whom 39 presented with synchronous and 36 with metachronous metastatic manifestations. For patients with synchronous metastases, an additional therapy of the thoracic locoregional disease with a curative intent (surgery, radiochemotherapy or definitive stereotactic radiotherapy) was required. All patients with metachronous metastases had a documented remission of the primary tumor. Ablative treatment of the complete extent of oligometastatic disease consisted of either complete surgical resection ± adjuvant radiotherapy or definitive radiotherapy with a biologically effective dose of 60 Gy. Endpoints of the analysis were median overall and progression-free survival. Results: Of the 75 patients, 57 presented with a solitary metastasis, in only 7 patients metastatic lesions were present in ≥ 2 organs and 66 patients had a Karnofsky performance score of 80 % or 90 %. The median follow-up was 54.0 months (95 % CI 28-81), the median overall survival 21.8 months (16.1-27.6) and the median progression-free survival 13.7 months ((9.7-17.6). In univariable Cox regression analysis, no single clinical factor was significantly associated with overall survival. For progression-free survival both ‘metastatic involvement of ≥ 2 organs vs. 1 organ’ (hazard ratio 0.43, 0.23-0.83, p=0.012) and a ‘Karnofsky performance score of 90 % vs. 70 % - 80 %’ (hazard ratio 4.32, 1.73-10.89, p=0.02) were significant prognostic factors as calculated by multivariable analysis. Comparing the cohorts with synchronous (n=39) vs. metachronous oligometastases (n =36), no differences in median overall and progression-free survival were found. Both cohorts were well-balanced except for the Karnofsky performance score, which was significantly superior in patients with synchronous oligometastases. Conclusions: Radical treatment of oligometastatic non-small-cell lung cancer was associated with acceptable long-term survival rates in patients with good Karnofsky performance score and it was equally effective for synchronous and metachronous manifestations.
