Abstract

Akrinor®, eine Cafedrin/Theodrenalin-Mischung (Caf/Theo), wird zur Therapie der perioperativen Hypotonie eingesetzt. Diese prospektive Observationsstudie untersuchte unter Verwendung kontinuierlicher Messverfahren den Einfluss von Caf/Theo auf Parameter der erweiterten Hämodynamik während Narkoseinduktion (n= 61, Caf-Dosis 0,8-1,0 mg/kg, Intervention bei mittlerem arteriellen Druck (MAP) <65 mmHg). Caf/Theo zeigte folgende pharmakodynamische Effekte: MAP-Anstieg (>65mmHg) in 52±43 s (MW±SD), Maximum (+52%) in 4,2±3,5 min; Peripherer Widerstandsindex (SVRI): Maximum (+51%) in 2,9±2,7 min; Herzindex (CI) Plateau (+18%) in 8,4±7,2 min; Schlagvolumenvariation (SVV) Minimum (-70%) in 9,2±10,4 min. Die Herzfrequenz (PR) blieb unverändert. Es zeigte sich ein signifikanter Einfluss des Geschlechts auf die Parameter MAP (p=0,045), CI (p=0,004) und PR (p=0,006), jedoch kein Einfluss einer β-Blocker-Vormedikation. Die schnelle und effektive antihypotensive Wirkung von Caf/Theo resultierte somit aus einem kombinierten Effekt auf SVRI, kardiale Vorlast und CI, die Parameter wiesen jedoch eine deutlich differierende Dynamik auf.

Extended pharmacodynamic effects of cafedrine/theodrenaline (Akrinor™) in anaesthesia-induced hypotension

Akrinor™, a cafedrine/theodrenaline mixture (Caf/Theo), is used to treat perioperative hypotension. This prospective observational study investigated the influence of Caf/Theo on parameters of extended haemodynamics during induction of anaesthesia using continuous measurement techniques (n=61, Caf dose 0.8-1.0 mg/kg, intervention at mean arterial pressure (MAP) <65 mmHg). Caf/Theo bolus showed the following pharmacodynamic effects: MAP increase (>65mmHg) in 52±43 s (MW±SD), maximum (+52%) in 4.2±3.5 min; peripheral resistance index (SVRI): maximum (+51%) in 2.9±2.7 min; cardiac index (CI): plateau (+18%) in 8.4±7.2 min; stroke volume variation (SVV): minimum (-70%) in 9.2±10.4 min. Heart rate (PR) remained unchanged. There was a significant influence of gender on the parameters MAP (p=0.045), CI (p=0.004) and PR (p=0.006), but no influence of β-blocker premedication. Repetative dosing of Caf/Theo showed no fading of haemodynamic effects. The rapid and effective antihypotensive effect of Caf/Theo thus resulted from a combined effect on SVRI, cardiac preload and CI, but these parameters showed considerably different dynamics.
