Abstract: Das aktive Knochenleitungsimplantat Bonebridge® zur Therapie der Schallleitungsschwerhörigkeit wurde 2012 vorgestellt und erhielt 2014 die Zulassung zur Implantation bei Kindern ab 5 Jahren. Da der Massenschwinger der Bonebridge® selbst bei Erwachsenen nicht immer in das Mastoid passt, stellt sich die Frage nach den Implantationsmöglichkeiten bei Kindern. CT-Scans von 151 kindlichen Mastoiden (5 Monate - 20 Jahre), sowie 52 adulte Mastoide wurden analysiert. Nach 3D-Rekonstruktion der Mastoide wurden Bonebridge® und ein noch nicht auf dem Markt verfügbares Knochenleitungsimplantat (Hakansson et al., 2010) mittels 3D-Konstruktionssoftware nach Herstellerangaben nachgebildet und deren Passfähigkeit durch virtuelle Implantation untersucht. Implantatdurchmesser und -höhe wurden anschließend modifiziert, um die vorteilhafteste Dimensionierung für Kinder zu ermitteln. Es wurden volumetrische und lineare Messungen am Mastoid durchgeführt, um das Knochenwachstum bei Kindern zu untersuchen. Darauf basierend wurde ein modifiziertes Implantatgehäuse entworfen und bei den Kindern virtuell implantiert.
The bone conduction implant Bonebridge® (introduced in 2012) for the treatment of conductive or mixed hearing loss of adults was approved for patients at the age of 5 years in 2014. Due to the size of the floating mass transducer, the Bonebridge does not fit in all adult mastoids. The possibility of fitting the implant in adults and children was investigated. CT-scans of 151 mastoids from children (5 months to 20 years) and 52 mastoids from adults were analysed. After 3D reconstruction of the mastoids, the Bonebridge® and a novel bone conduction implant (Hakansson et al., 2010) were reconstructed as provided by the manufacturers. The chance of fitting the implant into the mastoid bone was determined by virtual implantation. Implant diameter and depth were varied virtually to find the most advantageous dimension for children. Volumetric and linear measurements of mastoid bones were obtained to investigate mastoid growth. Based on that, a modified implant casing with reduced diameter and volume was designed and virtually implanted at children.
