Abstract: Eine erhöhte Thrombozytenzahl (Thrombozytose) stellt eine häufige Begleitstörung bei schwer kranken Patienten dar. Ziel dieser retrospektiven Beobachtungsstudie war es, die Ätiologie und die klinische Relevanz einer Plättchenzahl über 450x109/l bei Intensivpatienten zu analysieren. 307 Patienten mit intensivstationärer Mindestverweildauer von 4 Tage wurden in die Analyse eingeschlossen. Es wurde festgestellt, dass eine Thrombozytose mit Auftreten schwerer Komorbiditäten (z.B. SIRS und akute Hämorrhagie), mit der Dauer der Beatmungstherapie sowie mit einem erhöhten Risiko einer venösen Thromboembolie bei schwer kranken Patienten assoziiert ist. Sie verlängert die intensivmedizinische Verweildauer, senkt jedoch das intensivstationäre Mortalitätsrisiko. Demzufolge kann die Thrombozytose als ein Zusatzparameter in der Prognose- Vorhersage bei Intensivpatienten dienen.
An elevated platelet count (thrombocytosis) is a common finding in the critical ill patients. Aim of this retrospective observational study was to evaluate the aetiology and the clinical relevance of a platelet count greater than 450x109/l in the intensive care unit (ICU) patients. A total of 307 patients admitted to the ICU for at least 4 days were included in this study. Thrombocytosis was associated with severe complications (like SIRS and acute bleeding), with duration of mechanical ventilation as well as with the increased risk of the thromboembolism. It was also associated with the longer stay in the ICU but with the reduced risk of the ICU mortality. Therefore, the thrombocytosis may serve as an additional predictor of favourable outcome in ICU patients.
